FOR OVER 65 YEARS, NEW AVENUES HAS BEEN PROVIDING SAFE AND SUPPORTIVE ENVIRONMENTS WHERE WOMEN AND MEN WITH DISABILITIES CAN SIT BACK, RELAX, AND BE COMFORTABLE. THE INDIVIDUALS WE SERVE LIVE IN NINE INTERMEDIATE CARE FACILITIES, OR GROUP HOMES, OWNED AND OPERATED BY NEW AVENUES. IN ADDITION TO HAVING A FULLY STOCKED KITCHEN WITH DELICIOUS FOOD, A WARM BED TO SLEEP IN, AND STAFF TO SUPPORT THEM, EACH INDIVIDUAL SERVED BY NEW AVENUES IS ASSESSED TO DETERMINE WHAT SERVICES THEY NEED, AS WELL AS DESIRE. AN INDIVIDUAL HABILITATION PLAN (IHP) IS THEN TAILORED TO THAT INDIVIDUAL SO THAT HE OR SHE WILL BE ABLE TO REACH PERSONAL GOALS AND DEVELOP THE SKILLS NECESSARY FOR MORE INDEPENDENT LIVING. NEW AVENUES HAS A PROFESSIONAL STAFF AND CONSULTANTS WHO PROVIDE INDIVIDUALIZED SERVICES FOR EACH INDIVIDUAL SERVED INCLUDING A FULL ARRAY OF ASSISTIVE THERAPIES THAT BUILD INDEPENDENT LIVING SKILLS, INCLUDING SPEECH, ART, MUSIC, RECREATIONAL, AND PHYSICAL AND OCCUPATIONAL THERAPY. OUR CLINICAL SERVICES DEPARTMENT COMPRISED OF NINE NURSES IS CHARGED WITH PROVIDING COMPREHENSIVE CARE AND OVERSEEING THE HEALTH OF INDIVIDUALS SERVED.
THROUGH OUR COMMUNITY-BASED SERVICES PROGRAM, NEW AVENUES SERVES INDIVIDUALS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES, AGES 18 AND OLDER, WHO WANT RESIDENTIAL SUPPORTS FOR ACTIVITIES OF DAILY LIVING WITHIN THEIR HOME AND WHO WANT TO BE ACTIVELY ENGAGED IN THEIR COMMUNITIES. IN COMMUNITY-BASED SERVICES, OUR ROLE IS TO LISTEN TO THE PEOPLE WE SERVE AND CUSTOMIZE SERVICES TO ASSIST EACH PERSON IN ACHIEVING THEIR GOALS. ALL OF OUR SERVICES ARE FOCUSED ON ASSISTING INDIVIDUALS TO ATTAIN THE HIGHEST LEVEL OF INDEPENDENCE POSSIBLE - FROM WORKING WITH INDIVIDUALS TO CULTIVATE RELATIONSHIPS WITH THEIR HOUSEMATES, TO CONNECTING INDIVIDUALS TO OPPORTUNITIES FOR MEANINGFUL RELATIONSHIPS IN THE COMMUNITY. SERVICES ARE INTENDED TO FADE OVER TIME, AS THE PERSON SERVED DEVELOPS SKILLS AND SOCIAL CONNECTIONS THAT ENABLE THEM TO PARTICIPATE IN THEIR HOME AND COMMUNITY LIFE WITHOUT PAID SUPPORTS.
NEW AVENUES PROVIDES AN ARRAY OF INCLUSION SERVICES FOR ADULTS WITH DISABILITIES WHO WANT TO BE ACTIVELY ENGAGED IN THEIR COMMUNITIES. WE PROVIDE SUPPORT IN THE AREAS OF COMPETITIVE EMPLOYMENT AND COMMUNITY MEMBERSHIP. OUR INCLUSION SERVICES ARE CUSTOMIZED TO ASSIST EACH PERSON IN REACHING THEIR PERSONAL AND PROFESSIONAL GOALS. WE WORK WITH PEOPLE INDIVIDUALLY AND IN SMALL GROUPS (GENERALLY FOUR PEOPLE OR FEWER). ALL SERVICES ARE PROVIDED IN COMMUNITY SETTINGS THAT ARE ACCESSIBLE TO THE GENERAL PUBLIC. WE UTILIZE NON-DISABILITY-SPECIFIC RESOURCES, SUCH AS PUBLIC TRANSPORTATION, WHENEVER POSSIBLE. ALL SERVICES ARE FOCUSED ON ASSISTING EACH PERSON WE SERVE TO ATTAIN THE HIGHEST LEVEL OF INDEPENDENCE POSSIBLE. STAFF PROVIDE TEACHING AND COACHING SERVICES IN REAL LIFE SITUATIONS AND ASSIST INDIVIDUALS TO CULTIVATE RELATIONSHIPS WITH COMMUNITY MEMBERS.
