THE MISSION OF COMMUNITY ACCESS CENTER IS TO EMPOWER PERSONS WITH DISABILITIES TO CONTROL THEIR OWN LIVES, CREATE AN ACCESSIBLE COMMUNITY, AND ADVOCATE TO ACHIEVE COMPLETE SOCIAL, ECONOMIC, AND POLITICAL INTEGRATION.

TO SUPPORT CONSUMERS WITH DISABILITIES BY PROVIDING INFORMATION AND SERVICES FOR INDEPENDENT AND PRODUCTIVE LIVING. WE PROVIDED CASE COORDINATION FOR OVER 1,583 INDIVIDUALS CONSUMERS THIS YEAR. THERE WERE 2,732 MEASURABLE GOALS SET AND 2,521 SPECIFIC MEASURABLE GOALS ACCOMPLISHED, 56 INDIVIDUALS RECEIVED ADVOCACY SERVICES, 439 INDIVIDUALS RECEIVED ASSISTIVE TECHNOLOGY SERVICE, 286 INDIVIDUALS RECEIVED HOUSING SERVICES, 229 INDIVIDUALS RECEIVED INDEPENDENT LIVING AND LIFE SKILLS, 341 INDIVIDUALS RECEIVED PEER CONSOLING SERVICES, AND 4 INDIVIDUALS RECEIVED PERSONAL CARE SERVICES. A TOTAL OF 4 INDIVIDUALS TRANSITIONED FROM NURSING HOME TO PERSONAL RESIDENCES. DIVERSION SERVICE PREVENTED 75 INDIVIDUALS FROM BEING PLACE IN AN INSTITUTION. NUMBER OF INDIVIDUALS RECEIVED VOCATIONAL SERVICES AND TRANSITION AGE YOUTH, AGES 18 TO 26, 6. ALSO COMMUNITY ACCESS CENTER HAS OVER 2,859 COMMUNITY CONTACT THROUGH INFORMATION REFERRALS, OUTREACH PROGRAMS, AND PUBLIC EDUCATION. OUR WORK INCLUDED 7,963 HOURS OF COMMUNITY SERVICES AND 4,823 HOURS OF CASE COORDINATION
