THE AREA AGENCY ON AGING, PLANNING AND SERVICE AREA 2 IMPROVES THE QUALITY OF LIFE FOR THE OLDER ADULTS IN ITS NINE-COUNTY COMMUNITY BY ADVOCATING FOR INDIVIDUALS, DEVELOPING AND SUPPORTING A STRONG NETWORK OF SERVICES, AND PROVIDING RESOURCES FOR OLDER ADULTS.

OHIO DEPARTMENT OF AGING ADMINISTERED WAIVER PROGRAMS FOR PASSPORT AND ASSISTED LIVING: PASSPORT IS FOR SENIORS AGE 60 AND OLDER WHO NEED THE LEVEL OF CARE PROVIDED IN A NURSING HOME, BUT WANT TO STAY IN THEIR HOME. IT IS A STATEWIDE PROGRAM FUNDED BY MEDICAID.THOSE WHO QUALIFY FOR PASSPORT WORK WITH A NURSE OR SOCIAL WORKER TO DEVELOP A PLAN OF CARE THAT CUSTOMIZES SERVICES TO HELP THEM REMAIN AT HOME. ONCE ENROLLED, A CASE MANAGER WORKS WITH THEM TO BE SURE NEEDS ARE MET WITH QUALITY SERVICES, AND THAT THE PLAN OF CARE IS ADJUSTED AS NEEDS CHANGE. ASSISTED LIVING BRIDGES THE GAP BETWEEN INDEPENDENT LIVING AND NURSING HOME CARE. ASSISTED LIVING TYPICALLY OFFERS ASSISTANCE WITH THE ACTIVITIES OF DAILY LIVING AND ENSURES RESIDENTS HAVE ACCESS TO STAFF 24 HOURS A DAY. THE ASSISTED LIVING WAIVER PROGRAM IS ONE OF THE LONG-TERM CARE OPTIONS AVAILABLE STATEWIDE TO MEDICAID RECIPIENTS AND OFFERS AN ALTERNATIVE TO NURSING FACILITY PLACEMENT.
COMCARE IS FOR SENIORS WHO NEED SUPERVISION OR LIMITED ASSISTANCE TO REMAIN INDEPENDENT AT HOME. THE INTEGRATED CARE DELIVERY SYSTEM (ICDS) IS A SYSTEM OF MANAGED CARE PLANS SELECTED TO COORDINATE PHYSICAL, BEHAVIORAL, AND LONG-TERM CARE SERVICES FOR INDIVIDUALS OVER THE AGE OF 18 WHO ARE ELIGIBLE FOR BOTH MEDICAID AND MEDICARE. THIS INCLUDES PEOPLE WITH DISABILITIES, OLDER ADULTS, AND INDIVIDUALS WHO RECEIVE BEHAVIORAL HEALTH SERVICES. OHIO'S ICDS IS CALLED "MYCARE OHIO."
