MI HEALTH LINK - PROVIDES CARE COORDINATION AND SUPPORT SERVICES FOR MEMBERS OF INTEGRATED CARE ORGANIZATIONS UNDER THE STATE OF MICHIGAN DUAL ELIGIBLE PROGRAM. 3,314 SERVED.
