CHARTIERS OFFERS A WIDE RANGE OF MENTAL HEALTH SERVICES FOR INDIVIDUALS AND THEIR FAMILIES. THE PROGRAMS INCLUDE: OUTPATIENT SERVICES, PARTIAL HOSPITALIZATION, RESIDENTIAL TREATMENT (BOTH SHORT AND LONG TERM), SOCIAL REHABILITATION, BLENDED SERVICE COORDINATION, FAMILY AND HOUSING SUPPORT SERVICES, AND SUBSTANCE ABUSE SERVICES.
CHARTIERS OFFERS PROGRAMMING SUPPORT FOR INDIVIDUALS WITH INTELLECTUAL & DEVELOPMENTAL DISABILITIES. THE PROGRAMS INCLUDE: VOCATIONAL AND EMPLOYMENT SERVICES, SENIOR SERVICES, HOME & COMMUNITY HABILITATION AND A FAMILY LIVING PROGRAM.
WITH FUNDING FROM HUD, CHARTIERS CENTER OFFERS THE HESTIA, ATLAS, AND VESTA PROGRAMS. THE HESTIA AND ATLAS PROGRAMS SUPPORT CHRONICALLY HOMELESS INDIVIDUALS THAT ALSO EXPERIENCE A SEVERE MENTAL HEALTH DISORDER AND POSSIBLY A SUBSTANCE USE DISORDER/ADDICTION. THE PROGRAMS HELP INDIVIDUALS IDENTIFY THE HOUSING THEY WANT IN THE AREA THEY WISH TO LIVE. THE INDIVIDUALS DIRECTLY LEASE THE PROPERTY AND CHARTIERS USES HUD FUNDING TO PAY A PORTION OF THE INDIVIDUALS' RENT EXPENSES. OPERATING AS A SHELTER PLUS CARE MODEL, CHARTIERS SERVICE COORDINATION STAFF ALSO PROVIDE SUPPORT SERVICES AS NEEDED. THE VESTA PROGRAM SUPPORTS INDIVIDUALS BY HELPING IDENTIFY PERSONAL GOALS AND SELECTING AN APARTMENT OR HOUSE IN THE AREA OF THEIR CHOICE. FINANCIAL SUPPORT IS PROJECTED FOR ONE (1) YEAR AT WHICH TIME WE ANTICIPATE A SUCCESSFUL 'GRADUATION' TO STABILITY AND INDEPENDENCE.
