SENIOR COMPANION PROGRAM - TRAINED SENIOR COMPANIONS ASSIST SENIORS WITH ACTIVITIES OF DAILY LIVING WHICH ENABLE THEM TO REMAIN INDEPENDENT IN THEIR OWN HOME.
ADULT DAY CARE - FULL SERVICE PROGRAM FOR OLDER ADULTS IN NEED OF SUPERVISION, PERSONAL CARE, OR NURSING SERVICES. PARTICIPANTS SPEND THE DAY IN A CLEAN, SAFE AND STRUCTURED ENVIRONMENT.
DEMENTIA CARE/ RESPITE SERVICES - THROUGH RESPITE SERVICES, SOMEONE CARING FOR OLDER RELATIVES IS GIVEN THE OPPORTUNITY TO RUN ERRANDS OR PURSUE INTERESTS, KNOWING THAT THEIR LOVED ONE WILL RECEIVE PROPER CARE. TRAINED SENIOR COMPANIONS PROVIDE COMPANIONSHIP, HELPING HANDS AND UNDERSTANDING TO APPROXIMATELY 600 FRAIL ELDERLY IN LACKAWANNA COUNTY EACH YEAR.
TRANSPORTATION - PROVIDES APPROXIMATELY 8,000 RIDES PER YEAR TO MEDICAL APPOINTMENTS, SOCIAL SERVICES, ADULT DAY CARE, PHARMACIES, AND FOR GROCERIES TO ELIGIBLE FRAIL OLDER ADULTS, 60 YEARS OF AGE AND OLDER. COMPANION CARE PROGRAM - NON-MEDICAL ASSISTANCE FOR DEPENDENT OLDER ADULTS, INCLUDING DAILY LIVING TASKS. FRIENDLY VISITOR/ TEL-A-CARE PROGRAM - FACE TO FACE VISITS AND/ OR PHONE SAFETY CHECKS TO HOMEBOUND OLDER ADULTS. OUTREACH PROGRAM - PROVIDE FACE TO FACE INFORMATION ON PROGRAMS AND HELP COMPLETING BENEFIT APPLICATIONS.
