NUTRITION SERVICES: HOME DELIVERED MEAL SERVICE - HOME DELIVERED MEALS ARE PROVIDED TO OLDER ADULTS, AGED 60 OR OLDER, LIVING IN FAIRFIELD COUNTY. THESE MEALS ARE PREPARED AND DELIVERED DAILY, M-F, IN ACCORDANCE WITH THE OLDER ADULTS CARE PLAN. THEY ARE NUTRITIONALLY BALANCED AND APPROVED BY A REGISTERED DIETICIAN. CONGREGATE SITE MEAL SERVICE - CONGREGATE SITE MEALS ARE AVAILABLE TO OLDER ADULTS, AGE 60 OR OLDER, LIVING IN FAIRFIELD COUNTY. CONGREGATE MEAL SITES ARE OPEN DAILY, M-F, AND OFFER OLDER ADULTS THE OPPORTUNITY TO ENGAGE IN A SOCIAL SETTING WHILE RECEIVING A NUTRITIONALLY BALANCED MEAL. THE SITES REDUCE SENIOR ISOLATION AND HUNGER. NUTRITION TRANSPORTATION - TRANSPORTATION SERVICE TRANSPORTS AN OLDER ADULT, AGE 60 OR OLDER, LIVING IN FAIRFIELD COUNTY, FROM ONE PLACE TO ANOTHER THROUGH THE USE OF OUR AGENCY VEHICLES. WE PROVIDE NUTRITION TRANSPORTATION TO LUCYS CAFE CONGREGATE MEAL SITE AND TO GROCERY STORES. OUR SERVICE IS CURB TO CURB. AGING SERVICES: PERSONAL EMERGENCY RESPONSE SYSTEM - EMERGENCY RESPONSE SYSTEMS ARE PROVIDED TO OLDER ADULTS, AGE 60 OR OLDER, LIVING IN FAIRFIELD COUNTY. THESE UNITS ARE WORN BY THE OLDER ADULT AND ARE USED IN THE EVENT THAT THE OLDER ADULT HAS AN EMERGENCY. IN HOME SERVICES - PERSONAL CARE AND HOMEMAKING SERVICES ARE PROVIDED TO OLDER ADULTS, AGE 60 OR OLDER, LIVING IN FAIRFIELD COUNTY. THESE SERVICES ARE PROVIDED TO OLDER ADULTS WHO, DUE TO ILLNESS OR GENERAL PHYSICAL DECLINE, ARE NO LONGER ABLE TO SAFELY PROVIDE THEIR OWN PERSONAL CARE OR HOUSE CLEANING. CAREGIVER SUPPORT PROGRAM - THIS PROGRAM PROVIDES INDIVIDUALS UNCOMPENSATED CAREGIVERS FOR OLDER ADULTS, AGE 60 OR OLDER, LIVING IN FAIRFIELD COUNTY, BY PROVIDING SERVICES. THESE SERVICES CAN INCLUDE BUT ARE NOT LIMITED TO: HOME DELIVERED MEALS, PERSONAL CARE, HOMEMAKING, HOME REPAIR, IN-HOME RESPITE, ADULT DAY SERVICES. OTHER AGING SERVICES - INFORMATION AND REFERRAL, CASE MANAGEMENT, ADAPTIVE EQUIPMENT. SERVICE PROVIDER CONTRACTS - THE AGENCY CONTRACTS WITH LOCAL PROVIDERS TO DELIVER IN-HOME SUPPORT SERVICES TO OLDER ADULTS, AGE 60 OR OLDER, LIVING IN FAIRFIELD COUNTY.
