FROM ITS FACILITIES LOCATED AT 495, 497 AND 499 LESLIE STREET IN UKIAH, CALIFORNIA.THIS NONPROFIT HAS SERVED UKIAH'S SENIORS SINCE 1972, PROVIDING A WIDE VARIETY OF PROGRAMS, SERVICES AND ACTIVITIES THAT ENRICH AND IMPROVE THE LIVES OF COMMUNITY SENIORS AND DISABLED ADULTS, WHICH INCLUDE:TRANSPORTATION : THE TRANSPORTATION PROGRAM OPERATES THREE BUSES, FOUR DAYS PER WEEK, PROVIDING DOOR-THROUGH-DOOR RIDES TO LOCAL SENIORS AND DISABLED ADULTS IN THE SERVICE AREA. DURING FY 2018-2019, USC BUSES PROVIDED 7,166 RIDES, INCLUDING 1,490 FREE RIDES TO CAREGIVERS AND ESCORTS ACCOMPANYING CLIENT SENIORS AND DISABLED ADULTS. FOR MOST OF THESE RIDERS, THE USC BUSES ARE THE PRIMARY SOURCE OF LOCAL TRANSPORTATION TO HEALTHCARE APPOINTMENTS, EXCURSIONS FOR SHOPPING AND ERRANDS, AND TRIPS TO SOCIAL EVENTS OR ACTIVITIES AT THE CENTER.LUNCH BUNCH: THE LUNCH BUNCH PROGRAM IS THE ONLY ADULT PROGRAM IN THE UKIAH AREA THAT PROVIDES ACTIVITIES FOR ADULTS WITH DEMENTIA OR OTHER COGNITIVE IMPAIRMENTS.OPERATING FOUR DAYS A WEEK, LUNCH BUNCH OFFERS ITS CLIENTS AN OPPORTUNITY TO ENJOY A CATERED HOT LUNCH AND TO SOCIALIZE WITH OTHER ADULTS, PARTICIPATE IN VARIOUS GAMES AND ART PROJECTS, AND ENJOY MUSIC, DANCING AND EXERCISE ACTIVITIES. DURING FY 2018-2019, LUNCH BUNCH PROVIDED 1,711 ACTIVITY DAYS FOR ITS CLIENTS, INCLUDING 1,704 HOT MEALS AND 2,109 BUS RIDES FOR LUNCH BUNCH PARTICIPANTS. THIS PROGRAM PROVIDES A VITAL SERVICE FOR FAMILIES AND CAREGIVERS BY ALLOWING THEM TO HAVE A RESPITE FROM THEIR CARETAKER RESPONSIBILITIES WHILE THEIR FAMILY MEMBERS SAFELY ATTEND AN ENRICHING DAY PROGRAM SUITABLE TO THEIR NEEDS.EDUCATIONAL PROGRAMS SUCH AS DEFENSIVE DRIVING SCHOOLS, HICAP TRAINING, AND FRAUD PREVENTION WORKSHOPS.APPROXIMATELY 11% OF USC FUNDING IS PROVIDED THROUGH ENDOWMENTS AND GRANTS. GRANT MONIES HELP TO SUPPORT PROGRAMS SUCH AS LUNCH BUNCH AND TRANSPORTATION. CONTRACTS WITH MENDOCINO COUNTY HEALTH AND HUMAN SERVICES AND MENDOCINO TRANSIT AUTHORITY PROVIDE APPROXIMATELY 33% OF OUTREACH : THE USC OUTREACH PROGRAM PROVIDES ASSISTANCE TO SENIORS WITH DAILY LIVING ASSISTANCE NEEDS, SUCH AS MONEY MANAGEMENT SERVICES, CAREGIVER REFERRALS, TRANSPORTATION TO MEDICAL APPOINTMENTS OR PERSONAL ERRANDS, INSURANCE REFERRALS AND SOCIAL SERVICE AGENCY ASSISTANCE. OUTREACH MANGER HEATHER HAYDON WORKS CLOSELY WITH ADULT PROTECTIVE SERVICES TO PROVIDE REPRESENTATIVE PAYEE ASSISTANCE FOR LIMITED INCOME SENIORS WHO NEED HELP PAYING BILLS AND MANAGING THEIR FUNDS. HEATHER AND HER OUTREACH ASSISTANT ALSO PROVIDE LUNCHES TO HOMEBOUND SENIORS WHO NEED MEAL ASSISTANCE. OUTREACH STAFF AND OUTREACH VOLUNTEERS ALSO PROVIDE RIDES FOR SENIORS TO OUT-OF-TOWN MEDICAL APPOINTMENTS THAT ARE OUTSIDE OF THE SERVICE AREA FOR USC BUSES. USC OUTREACH STAFF AND VOLUNTEERS DROVE OVER 5,000 MILES DURING FY 2018-2019 ASSISTING HOMEBOUND SENIORS WITH HEALTHCARE RIDES, DELIVERY OF COMMODITIES AND MEALS, AND RUNNING PERSONAL ERRANDS.BARTLETT HALL SOCIAL ACTIVITIES: THE PRIMARY COMMUNITY CENTER AT USC IS BARTLETT HALL, WHICH ACCOMMODATES 166 ADULTS FOR DINING ACTIVITIES.EQUIPPED WITH A FULL COMMERCIAL KITCHEN, BARTLETT HALL PROVIDES AN AFFORDABLE HOT CATERED LUNCHEON FIVE DAYS A WEEK THAT IS OPEN TO ALL SENIORS AND DISABLED ADULTS. ALL MONTHLY MENUS ARE DISPLAYED IN THE CENTER'S NEWSLETTER, THE SCOOP, WHICH IS AVAILABLE ONLINE AT THE USC WEBSITE, WWW.UKIAHSENIORCENTER.ORG. A WEEKLY MEAL TICKET FOR FIVE MEALS CAN BE PURCHASED FOR ONLY $30. DURING FY 2017-2018, BARTLETT HALL SERVED 4,607 MEALS, INCLUDING TAKE-OUT MEALS FOR HOMEBOUND SENIORS. A MONTHLY BIRTHDAY LUNCHEON IS HELD ON THE THIRD FRIDAY OF EACH MONTH TO HONOR THOSE SENIORS WHO ARE CELEBRATING A BIRTHDAY DURING THE MONTH.BARTLETT HALL IS THE SITE FOR MOST OF USC'S PROGRAMS AND ACTIVITIES, INCLUDING:WEEKLY FRIDAY MOVIE MATINEE SHOWINGS, WHICH IS FREE TO ALL SENIORS;WEEKLY BINGO GAMES ON TUESDAYS AND THURSDAYS, AND SPECIAL BINGO BASH EVENTS ON DESIGNATED SUNDAYS;WEEKLY BRIDGE AND PINOCHLE GAMES;MONTHLY BUNKO EVENTS;WEEKLY ACTIVITIES SUCH AS
