COMMUNITY CARE PROGRAM: THE ILLINOIS DEPARTMENT ON AGING'S SUBSIDIZED IN-HOME CARE PROGRAM PROVIDES A VARIETY OF SERVICES FOR OLDER ADULTS WHO NEED ASSISTANCE WITH ACTIVITIES OF DAILY LIVING ON A REGULAR BASIS. ELIGIBILITY FOR THIS PROGRAM IS BASED ON THE NEED FOR CARE AND FINANCIAL CRITERIA. A CARE COORDINATOR CAN PROVIDE AN IN-HOME ASSESSMENT TO DETERMINE ELIGIBILITY FOR THIS PROGRAM. SERVICES PROVIDED BY THE COMMUNITY CARE PROGRAM INCLUDE: HOMEMAKERS SERVICES, ADULT DAY CARE, AUTOMATED MEDICATION DISPENSER, AND EMERGENCY HOME RESPONSE SYSTEMS.
AGING & DISABILITY RESOURCE NETWORK: SOLUTIONS FOR CARE IS A STATE-DESIGNATED AGING & DISABILITY RESOURCE NETWORK (ADRN) SITE THAT PROVIDES UP-TO-DATE INFORMATION AND QUALITY REFERRALS TO INDIVIDUALS WITH DISABILITIES AGES 18-59 AND TO OLDER ADULTS AND THEIR FAMILY MEMBERS. ADRN ALSO OFFERS THE SENIOR HEALTH ASSISTANCE PROGRAM (SHAP) WHICH CONNECTS OUR CLIENTS WITH STATE AND FEDERAL PROGRAMS. ADRN CONDUCTS SERVICES AT OUR MAIN OFFICE IN NORTH RIVERSIDE MONDAYS-FRIDAYS. THE AGENCY ALSO HOSTS ENROLLMENT EVENTS AT ALTERNATE LOCATIONS THROUGHOUT THE YEAR.
ADULT PROTECTIVE SERVICES: SOLUTIONS FOR CARE IS A STATE-DESIGNATED ADULT PROTECTIVE SERVICES (APS) AGENCY THROUGH THE ILLINOIS DEPARTMENT ON AGING WHICH INVESTIGATES ABUSE, NEGLECT, FINANCIAL EXPLOITATION, AND SELF-NEGLECT OF INDIVIDUALS AGES 18-59 LIVING WITH A DOCUMENTED DISABILITY AND ANYONE AGE 60 AND OVER. ALL ADULT PROTECTIVE SERVICES ARE CONFIDENTIAL AND FREE OF CHARGE. THE SOLUTIONS FOR CARE APS TEAM SERVES BERWYN, CICERO, AND PROVISO TOWNSHIP. INVESTIGATING APPROXIMATELY 400 CASES ANNUALLY, WITH FINANCIAL EXPLOITATION BEING THE MOST FREQUENT OFFENSE.
