CARE COORDINATION. OUR CARE COORDINATORS SCHEDULED 3351 SPECIALTY CARE APPOINTMENTS AND PLACED AND ANSWERED 79,750 PHONE CALLS, EMAILS AND RECORD ENTRIES IN SERVICE TO THOSE APPOINTMENTS. WE REDUCED THE COSTS OF THE CARE FOR OUR PARTNERS, INCREASED ACCESS TO SERVICES, AND ENSURED PEOPLE WERE RETURNED TO HEALTH, THEIR FAMILIES AND THEIR COMMUNTIES.
PREMIUM ASSISTANCE. WITH PARTNER SUPPORT, WE PURCHASE HEALTH INSURANCE ON THE EXCHANGE FOR THOSE WHO CANNOT OTHERWISE AFFORD THE PREMIUMS. THIS ALLOWS PATIENTS TO ACCESS ONGOING, APPROPRIATE HEALTH CARE RATHER THAN EMERGENCY CARE.
