NJ SUPPORT COORDINATION: USING A PERSON-CENTERED TEAM APPROACH, SUPPORT COORDINATION IS A CASE MANAGEMENT COMPONENT OF THE SUPPORTS PROGRAM, AND COMMUNITY CARE PROGRAM FOR PEOPLE WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES. THIS SERVICE IS DESIGNED TO HONOR THE HOPES AND DREAMS OF INDIVIDUALS AND THEIR FAMILIES BY FACILITATING A UNIQUE AND INDIVIDUALIZED PLANNING PROCESS THAT ENABLES CONSUMERS TO MAKE INFORMED CHOICES ABOUT ELIGIBLE SERVICES AND SUPPORTS. NJ SUPPORT COORDINATION IS FUNDED BY MEDICAID FEE-FOR-SERVICES IN THE COMPREHENSIVE WAIVER.
