PERSONAL HOME CARE SERVICES TO ELDERLY AND/OR DISABLED PERSONS ELIGIBLE FOR MEDICAID BENEFITS AND ENROLLED UNDER SERVICES CONTRACT WITH CITY OF NEW YORK HRA. THE ORGANIZATION PROVIDED NURSING ASSESSMENT AND RELATED NURSING SERVICES TO APPROXIMATELY 128 CLIENTS.
PROVIDING QUALITY COMPASSIONATE AND SUPPORTIVE SERVICES TO ELDERLY AND DISABLED NEW YORK CITY RESIDENTS UNDER AGREEMENTS WITH MANAGED CARE ORGANIZATIONS AND MANAGED CARE LONG TERM CARE ORGANIZATIONS.
