PERSONAL HOME CARE SERVICES TO ELDERLY AND/OR DISABLED PERSONS ELIGIBLE FOR MEDICAID BENEFITS AND ENROLLED UNDER SERVICES CONTRACT WITH CITY OF NEW YORK HRA. THE ORGANIZATION PROVIDED NURSING ASSESSMENT AND RELATED NURSING SERVICES TO APPROXIMATELY 162 CLIENTS.
