COMMUNITY ASSESSMENT AND TRANSPORT TEAM (CATT): THE COMMUNITY ASSESSMENT AND TRANSPORT TEAM (CATT) IS AN INNOVATIVE, COUNTY-WIDE PROGRAM DEVELOPED IN COLLABORATION WITH ALAMEDA COUNTY BEHAVIORAL HEALTH, ALAMEDA COUNTY CARE CONNECT, ALAMEDA COUNTY EMERGENCY MEDICAL SERVICES, AND FALCK 911 EMERGENCY RESPONSE SERVICES. CATT IS A MOBILE RESPONSE PROGRAM OPERATING OUT OF A FLEET OF MODIFIED SUVS. EACH VEHICLE IS STAFFED BY A BONITA HOUSE LICENSED BEHAVIORAL HEALTH CLINICIAN AND AN EMERGENCY MEDICAL TECHNICIAN (EMT). THE CATT TEAMS ARE DEPLOYED TO SPECIFIC LOCATIONS TO PARTNER WITH LAW ENFORCEMENT BASED ON 911 CALLS SCREENED BY DISPATCH THAT INVOLVE BEHAVIORAL HEALTH AND/OR SUBSTANCE USE EMERGENCIES. THE CATT TEAMS ARE ABLE TO PROVIDE ON-SCENE MENTAL HEALTH AND MEDICAL ASSESSMENTS, CRISIS MANAGEMENT, AND BASIC MEDICAL CARE IF NEEDED. THEY CAN ALSO INITIATE A REFERRAL, PLACE INDIVIDUALS ON A 5150 HOLD, OR TRANSPORT INDIVIDUALS WITH NO OBSERVABLE MEDICAL EMERGENCY TO ANOTHER LOCATION SUCH AS A SHELTER, SOBERING CENTER, WELLNESS CENTER, MENTAL HEALTH FACILITY, OR ANY OTHER DESIGNATED DESTINATION APPROPRIATE TO MEET THE CLIENT'S IMMEDIATE NEEDS.
BRIDGES TO RECOVERY/IHOT:THE BRIDGES TO RECOVERY IN-HOME OUTREACH TEAM (IHOT) PROGRAM PROVIDES SHORT-TERM OUTREACH AND ENGAGEMENT TO INDIVIDUALS WITH SERIOUS, UNTREATED MENTAL ILLNESS IN AN EFFORT TO LINK THEM TO BEHAVIORAL HEALTHCARE AND OTHER COMMUNITY SERVICES TAILORED TO THEIR COMPLEX NEEDS. THE IHOT TEAM USES A TRAUMA INFORMED, SENSITIVE, AND CULTURALLY RESPONSIVE APPROACH THAT CONSIDERS THE CLIENT AND THE FAMILY'S PACE OF CHANGE. FAMILY MEMBERS ARE PROMINENTLY INCLUDED IN ENGAGEMENT AND PLANNING, AND ARE ENCOURAGED TO PARTICIPATE IN A VARIETY OF SUPPORTS TO ASSIST THEM AND THEIR FAMILY MEMBERS, WHILE ADDRESSING THEIR OWN NEEDS AND STRESSORS. RESIDENTIAL TREATMENT:THE DUAL-DIAGNOSIS RESIDENTIAL TREATMENT PROGRAM IS UNIQUELY DESIGNED TO SERVE UP TO 15 ADULTS LIVING WITH BOTH SERIOUS PSYCHIATRIC AND SUBSTANCE USE DISORDERS IN A 24-HOUR RESIDENTIAL SETTING. BONITA HOUSE, FOUNDED IN 1971, WAS THE FIRST TREATMENT PROGRAM OF ITS KIND IN BERKELEY, CA. BONITA HOUSE INC.'S STAFF IS CROSS-TRAINED IN EVIDENCE BASED INTEGRATED DUAL DIAGNOSIS TREATMENT (IDDT) PRACTICES THAT ADDRESS BOTH METAL HEALTH AND SUBSTANCE USE INTERVENTION, TREATMENT, AND RECOVERY. BONITA HOUSE HAS BEEN CONSIDERED A LEADER IN THE FIELD OF DUAL-DIAGNOSIS TREATMENT, BOTH LOCALLY AND NATIONALLY, FOR OVER 30 YEARS.SERVICES ARE PERSON-CENTERED AND COLLABORATIVE, AND INCLUDE ONGOING ASSESSMENT, EVALUATION, AND STABILIZATION; AN INTENSIVE DAY TREATMENT PROGRAM; DUAL-DIAGNOSIS RECOVERY AND OTHER GROUPS; AND INDIVIDUAL MENTAL AND BEHAVIORAL HEALTH COUNSELING. THE FOCUS IS ON ENABLING RESIDENTS TO BUILD THE SKILLS NECESSARY TO INCREASE THEIR ABILITY TO FUNCTION IN THE LIVING, LEARNING, WORKING, AND SOCIAL ENVIRONMENT OF THEIR CHOICE.CALWORKS PHOENIX SKILLS:THE CALWORKS (CALIFORNIA WORK OPPORTUNITY AND RESPONSIBILITY TO KIDS) PHOENIX SKILLS PROGRAM PROVIDES SERVICES FOR CLIENTS AND THEIR FAMILIES EXPERIENCING MENTAL HEALTH CHALLENGES THAT CREATE BARRIERS TO EMPLOYMENT AND EDUCATION. THE PHOENIX SKILLS PROGRAM UTILIZES A MULTIDISCIPLINARY APPROACH TO IMPROVE THE COPING ABILITIES, CONCENTRATION, MOTIVATION, AND SELF-ESTEEM OF CLIENTS. SERVICES INCLUDE MENTAL HEALTH AND SUBSTANCE USE COUNSELING; HELP WITH IDENTIFYING STRENGTHS AND WORK INTERESTS; GUIDANCE THROUGH THE PROCESS THAT LEADS TO A JOB INTERVIEW; INFORMATION AND ACCESS TO COMMUNITY RESOURCES TO SUPPORT AND MAINTAIN MENTAL HEALTH; AND SUPPORT FOR GAINING OR FURTHERING EDUCATION. THE CALWORKS TEAM SUPPORTS CLIENTS AND THEIR FAMILIES TO ATTAIN SELF-SUFFICIENCY AND THEIR FULLEST POTENTIAL.
THE WELLNESS CENTERS PROVIDE AN ARRAY OF SERVICES AT LOCATIONS IN EAST OAKLAND AND BERKELEY. EACH PROGRAM IS DESIGNED TO PROVIDE SOCIALIZATION IN A CARING COMMUNITY THAT FOSTERS SELF-SUFFICIENCY FOR INDIVIDUALS LIVING WITH SERIOUS MENTAL HEALTH CHALLENGES. THE BERKELEY WELLNESS CENTER (BWC) IS AN ARTS-BASED PROGRAM, AND CASA UBUNTU (WHICH MEANS "HOUSE OF HUMAN KINDNESS") OFFERS SERVICES IN ENGLISH AND SPANISH. THE CENTERS' STRENGTH-BASED PROGRAMS INCLUDE PEER-TO-PEER SUPPORT GROUPS, EXPRESSIVE ART ACTIVITIES, INDIVIDUAL MENTAL HEALTH SERVICES, RESOURCE REFERRALS, AND ENCOURAGE PARTICIPANTS TO DESIGN THEIR OWN WELLNESS PROGRAMS BASED ON THEIR PERSONAL INTERESTS, NEEDS AND GOALS.
SUPPORTED INDEPENDENT LIVING (SIL):THE SUPPORTED INDEPENDENT LIVING (SIL) PROGRAM SERVES UP TO 166 INDIVIDUALS WITH A COMPREHENSIVE RANGE OF SERVICES AND HOUSING OPTIONS. THE SIL CASE MANAGEMENT SERVICES TEAM PROVIDES INTENSIVE OUTPATIENT MENTAL HEALTH AND CASE MANAGEMENT SERVICES TO INDIVIDUALS WITH CO-OCCURRING SERIOUS MENTAL ILLNESS AND SUBSTANCE USE DISORDERS LIVING IN ALAMEDA COUNTY. THE PROFESSIONAL STAFF ALSO PROVIDES PSYCHIATRIC MEDICATION SUPPORT AND ASSESSMENT, AND EVIDENCE-BASED BEHAVIORAL REHABILITATION SERVICES TO SUPPORT RECOVERY AND ENABLE CLIENTS TO MAINTAIN BASIC EVERYDAY NEEDS, INCLUDING HOUSING AND FINDING MEANINGFUL ROLES IN THE COMMUNITY. THE SIL HOUSING COMPONENT PROVIDES LONG-TERM PERMANENT HOUSING TO LOW-INCOME ADULTS WHO ARE HOMELESS, OR AT RISK OF HOMELESSNESS, AND LIVING WITH SERIOUS MENTAL HEALTH CHALLENGES.
