PROVIDE TRANSPORTATION TO AND FROM THE CENTER FOR CONGREGATE MEALS AND CENTER ACTIVITIES. PROVIDE TRANSPORTATION TO NECESSARY MEDICAL APPOINTMENTS AND ADULT DAY SERVICES
IN-HOME SERVICES: PROVIDES RESPITE SERVICES, PROVED IN-HOME CAREGIVING TO THE ELDERLY, AND CHORE SERVICES. KEEPING THE PHYSICAL ENVIRONMENT CLEAN, SAFE AND HEALTHY FOR THOSE INDIVIDUALS. NATIONAL FAMILY CAREGIVER: FOCUSES ON THE NEEDS OF THE FAMILY CAREGIVERS.
