CANCER SCREENING REGISTRY FOR COLORECTAL CANCER: TO REDUCE REDUNDANCY AND STREAMLINE REPORTING BY CREATING A SINGLE STATE-WIDE CANCER SCREENING REGISTRY FOR LOUISIANA SO MORE EFFICIENT ACCESS IS AVAILABLE TO RELIABLE POPULATION BASE SCREENING DATE FOR COLORECTAL AND BREAST CANCER.


INCREASING COLORECTAL CANCER SCREENING IN PRIMARY CARE PRACTICES: ESTABLISHED SYSTEMS THAT SUPPORT OFFICE PRACTICES THAT WILL RESULT IN RELIABLE AND SUSTAINABLE SCREENING AND FOLLOW-UP SYSTEMS FOR CRC SCREENING TO INTIATION OF TREATMENT IN FEDERALLY QUALIFIED HEALTH CENTERS AND RURAL HEALTH CENTERS. TACL HAS CONTRACTED WITH THE LOUISIANA CANCER CONTROL AND PREVENTION PROGRAM (LCP) TO IMPLEMENT A LEARNING COLLABORATIVE WITH 5 IDENTIFIED PRIMARY CARE PRACTICES WHO SERVE A MAJORITY OF MEDICAL PATIENTS TO HELP THEM IMPLEMENT EVIDENCE-BASED INTERVENTIONS TO INCREASE THEIR CURRENT COLORECTAL CANCER SCREENING RATES. THIS WILL BE A VIRTUAL LEARNING SERIES OVER 12 MONTHS THAT WILL BRING CLINICIANS TOGETHER TO LEARN BEST PRACTICES THAT WILL HELP SET-UP RELIABLE SYSTEMS THROUGHOUT THE SCREENING PROCESS. TACLS ROLL WILL BE TO WORK WITH LCP TO OVERSEE THE PLANNING AND INTERVENTION. LCP WILL ULTIMATELY BE RESPONSIBLE FOR THE IMPLEMENTATION.


LOUISIANA PAYER COUNCIL: TO ALIGN PAYER SYSTEMS AROUND TACLS GOALS AND PRIORITIES AND COORDINATE AND ALIGN EFFORTS TO REMOVE BARRIERS TO COLORECTAL CANCER SCREENING.
