COMMUNITY HEALTH WORKER (CHW)-PATIENT NAVIGTOR (PN) LEARNING COLLABORATIVE: TRAINING EXISTING CHW-PNS ON CANCER COMPONENTS FOR BREAST AND COLORECTAL CANCERS TO HELP OVERCOME BARRIERS FOR GETTING INDIVIDUALS SCREENED IN POOR AREAS. TACL LAUNCHED A 11 MONTH VIRTUAL LEARNING COLLABORATIVE WITH COMMUNITY HEALTH WORKERS AND PATIENT NAVIGATORS TO TEACH THEM HOW TO EDUCATE AND MOVE INDIVIDUALS THROUGH THE COMPLETE CONTINUUM OF CARE FROM SCREENING TO INTIATION OF TREATMENT FOR BREAST AND COLORECTAL CANCER. WE HAD APPROXIMATELY 20 CHW-PNS FROM MANAGED CARE ORGANIZATIONS, LOUIANA DEPARTMENT OF HEALTH, LOUISIANA PRIMARY CARTE SYSTEMS, PREVENTION ON THE GO SCREENING MOBILE PROGRAM (MBP CANCER CENTER) AND PARTNERS IN WELLNESS MOBILE SCREENING PROGRAM IN NORTH LOUISIANA.
LOUISIANA PAYER COUNCIL: TO ALIGN PAYER SYSTEMS AROUND TACLS GOALS AND PRIORITIES AND COORDINATE AND ALIGN EFFORTS TO REMOVE BARRIERS TO COLORECTAL CANCER SCREENING.
