CASE MANAGEMENT - WE COORDINATE NEEDED COMMUNITY-BASED SERVICES SO THAT INDIVIDUALS WHO ARE NURSING HOME LEVEL OF CARE AND MEDICAID ELIGIBLE CAN REMAIN IN THEIR HOMES INSTEAD OF NURSING HOMES. WE FACILITATE THE TRANSITION OF NURSING HOME RESIDENTS INTO THEIR OWN HOMES. WE SERVED 337 INDIVIDUALS, INCLUDING ONE TRANSITION FROM A NURSING HOME TO THE COMMUNITY.
INDEPENDENT LIVING (IL) SERVICES - IL SERVICES HELP INDIVIDUALS GAIN EFFECTIVE CONTROL AND DIRECTION IN THEIR LIVES. SERVICES INCLUDE INFORMATION AND REFERRAL, PEER COUNSELING, INDIVIDUAL AND SYSTEM ADVOCACY, SKILLS TRAINING, DIABETES EDUCATION PROGRAM, TAI CHI FOR ARTHRITIS FOR FALL PREVENTION, BINGOCIZE, DONATIONS OF USED DURABLE MEDICAL EQUIPMENT, HEALTH AND WELLNESS EDUCATION, AND HELP WITH ACCESSING HEALTH CARE SERVICES INCLUDING COVID-19 VACCINES. WE RESPONDED TO 2,426 REQUESTS FOR INFORMATION AND REFERRAL, DONATED DURABLE MEDICAL EQUIPMENT TO 108 INDIVIDUALS AND PROVIDED HYGIENE PRODUCTS TO 24 INDIVIDUALS. WE PROVIDED HOLIDAY MEALS TO 11 FAMILIES. THREE HUNDRED NINETY-SEVEN SENIORS PARTICIPATED IN AT LEAST ONE HEALTH PROMOTION WORKSHOP, AND 91 COMPLETED 16 HOURS OF TAI CHI TRAINING AND REPORTED IMPROVED BALANCE AND MUSCLE STRENGTH TO REDUCE FALLS. WE HAD 26 PARTICIPANTS IN OUR DIABETES EMPOWERMENT AND EDUCATION PROGRAM. SIXTEEN PARTICIPANTS COMPLETED FIVE OR MORE OF THE SIX DIABETES CLASSES AND REPORTED THEY GAINED PRACTICAL SKILLS IN MANAGING DIABETES, INCREASED THEIR KNOWLEDGE OF DIABETES AND ITS RISK FACTORS, INCREASED SELF-MONITORING SKILLS, AND LEARNED HOW TO USE MEDICATIONS APPROPRIATELY. BINGOCIZE (THE FAMILIAR GAME OF BINGO COMBINED WITH EXERCISE AND HEALTH NUTRITION) HAD 88 PARTICIPANTS. NINETEEN PARTICIPANTS COMPLETED 16 HOURS OR MORE OF THE BINGOCIZE TRAINING.
