MONARCHCARE PROVIDES QUALITY CARE MANAGEMENT, GUARDIANSHIP AND PERSONALIZED FINANCIAL SERVICES TO AT-RISK INDIVIDUALS ACROSS SOUTH FLORIDA REGARDLESS OF THE CLIENTS' ABILITY TO PAY. CLIENTS RANGE FROM MINOR CHILDREN TO THE FRAIL ELDERLY, FROM MENTALLY ILL YOUTHS TO ADULTS WITH DEVELOPMENTAL DISABILITIES. CHALLENGES FACED BY OUR CLIENTS INCLUDE: -INABILITY TO MANAGE DAILY NEEDS DUE TO DECLINE IN PHYSICAL HEALTH OR COGNITIVE FUNCTIONING, AND/OR MULTIPLE MEDICAL ISSUES. -EASILY CONFUSED, VULNERABLE, AND AT-RISK FOR ABUSE, EXPLOITATION AND/OR SELF-NEGLECT. -DIFFICULTY NAVIGATING THE COMPLEX HEALTHCARE SYSTEM. MONARCHCARE IS RECOGNIZED AS A LEADER IN FILLING THE GAPS IN SERVICES FOR SOUTH FLORIDA'S VULNERABLE CITIZENS. OVER THE YEARS, MONARCHCARE HAS RESCUED SUSCEPTIBLE INDIVIDUALS FROM FINANCIAL EXPLOITATION, RESTORED INSTITUTIONALIZED PERSONS TO INDEPENDENT LIFESTYLES AND RECONNECTED ISOLATED PERSONS WITH LONG LOST FAMILY AND FRIENDS. IN FISCAL YEAR 2021- 2022, MONARCHCARE SERVED OVER 107 CLIENTS, PROVIDING INTENSIVE CASE MANAGEMENT, PERSONALIZED FINANCIAL AND GUARDIANSHIP SERVICES. DURING THIS TIME, MANY OF OUR CLIENTS WERE AT GREAT RISK FOR COVID-19. WE ARE PROUD TO STATE WE ONLY LOST TWO OF OUR CLIENTS TO THE DEADLY DISEASE, BOTH AT THE VERY ONSET OF THE PANDEMIC. MONARCHCARE PROGRAMS AND SERVICES ENSURE OUR CLIENTS HAVE A STABLE AND SUPPORTIVE LIFESTYLE WITHIN THE LEAST RESTRICTIVE ENVIRONMENT. MONARCHCARE SERVES SOME CLIENTS FOR A LIFETIME. WE HAVE THREE INTER-RELATED PROGRAMS THAT PROVIDE DIRECT SERVICES TO CLIENTS AND A FOURTH SERVICE OFFERING COMMUNITY EDUCATION. OUR CARE MANAGEMENT PROGRAM PROVIDED ALMOST 1,500 HOURS OF INTENSE CARE MANAGEMENT TO AT-RISK INDIVIDUALS, INCLUDING PERSONS WHO ARE LEGALLY INCAPACITATED. OUR CARE MANAGERS PROVIDED REGULAR CLIENT VISITS AT THEIR HOMES, FACILITIES AND DOCTOR'S OFFICES TO ENSURE THAT PROGRAM PARTICIPANTS RECEIVED ALL NEEDED SERVICES ALONG WITH DIGNIFIED AND COMPASSIONATE TREATMENT FROM OTHER SERVICE PROVIDERS. INDIVIDUALS WHO BENEFIT FROM CARE MANAGEMENT LIVE ALONE, HAVE NO FAMILY OR SUPPORT SYSTEM NEARBY, HAVE MULTIPLE MEDICAL ISSUES, POOR NUTRITION, OR ARE EASILY INFLUENCED OR CONFUSED. THIS YEAR WE PROVIDED OVER 1,900 HOURS OF GUARDIANSHIP SERVICES TO INCAPACITATED INDIVIDUALS, MANY OF WHOM ALSO RECEIVE OUR CARE MANAGEMENT SERVICES. IF AN INDIVIDUAL IS LEGALLY INCAPACITATED, HAS NO ADVANCE DIRECTIVES, AND IS NON-COMPLIANT WITH THE NECESSARY ASSISTANCE TO PROVIDE FOR THEIR WELL-BEING, THEN GUARDIANSHIP MAY BE THE ONLY ALTERNATIVE TO KEEP HIM OR HER SAFE. BECAUSE GUARDIANSHIP REMOVES A PERSON'S RIGHTS, IT IS USED ONLY AS A LAST RESORT, TO PROTECT THE MOST VULNERABLE OF INDIVIDUALS.
