MEDICAID WAIVER RESIDENTIAL, DAY, AND PERSONAL ASSISTANCE. RESIDENTIAL - THIS INCLUDES BOTH GROUP HOMES AND SUPPORTING LIVING IN FOUR COUNTIES. THIRTY EIGHT INDIVIDUALS ARE SERVED. ACHIEVEMENTS ARE RELATED TO COMMUNITY ACTIVITIES AND DAILY LIVING SKILLS. DAY - THIS INCLUDES SERVICES TO INDIVIDUALS IN PREVOCATIONAL TRAINING CENTERS, COMMUNITY BASED ACTIVITIES, OPPORTUNITIES FOR WORK, AND SERVES APPROXIMATELY ONE HUNDRED AND FIFTEEN INDIVIDUALS. ACHEIVEMENTS ARE RELATED TO PROVIDING OVERSIGHT/TRASPORTATION FOR PHYSICIANS/APPOINTMENTS AND PROVIDING LEISURE AND LEARNING ACTIVITIES.
