END OF LIFE CARE, COMFORT AND SUPPORT FOR PATIENTS AND THEIR FAMILIES. THE CARE IS PROVIDED IN PATIENT'S HOMES, ASSISTED LIVING FACILITIES, NURSING HOMES, HOSPITALS AND HOPE HOSPICE HOUSES. MEDICAL AND OTHER CARE PROFESSIONALS ARE DEDICATED TO MEETING THE NEEDS OF EACH PERSON. THE ORGANIZATION PROVIDES SERVICES TO LEE, HENDRY, AND GLADES, COUNTIES. (AVERAGE DAILY CENSUS OF 1,094 PATIENTS FOR FISCAL YEAR 2020-21).
