CASE MANAGEMENT:CASE MANAGERS COORDINATE WITH SENIORS (AND THEIR CAREGIVERS) TO IDENTIFY AND ARRANGE FOR THE SERVICES NEEDED TO SUPPORT THE INDIVIDUAL IN THEIR HOME. IN HOME SUPPORT INCLUDES, COMPANIONSHIP, HOMEMAKING, PERSONAL CARE, MEALS AND RESPITE. 131 OLDER ADULTS RECEIVED IN-HOME SERVICES; 100% OF CASE MANAGED CLIENTS WERE DEFERRED FROM FACILITY PLACEMENT.90 HOUSEHOLDS WERE ASSISTED THROUGH THE EMERGENCY HOME ENERGY ASSISTANCE FOR THE ELDERLY PROGRAM.INFORMATION AND REFERRAL FOR PROGRAMS AND SERVICES ARE AVAILABLE TO SENIORS. APPROXIMATELY 2,324 INDIVIDUALS RECEIVED ASSISTANCE THROUGH INFORMATION AND REFERRAL.
DIRECT ELDERLY CARE ADULT DAY CARE - PROVIDES COORDINATED HEALTH, SOCIAL, AND THERAPEUTIC ACTIVITIES IN A SUPERVISED GROUP SETTING FOR ADULTS WITH FUNCTIONAL AND/OR COGNITIVE IMPAIRMENTS. 96 UNDUPLICATED CLIENTS SERVED; 81% DEFERRED FROM NURSING HOME PLACEMENT.
