COMMUNITY COACH: 41,189 ONE WAY TRIPSGOLINEPUBLIC TRANSPORTATION SERVICE ON 14 FIXED ROUTES IN THE COUNTY FOR RIDERS 12 YEARS OR OLDER, RIDERS UNDER 12 MUST BE ACCOMPANIED BY AN ADULT. GOLINE: 1,135,856 ONE-WAY TRIPS

THE PROGRAM GOALS ARE TO PREVENT HOSPITAL OR FACILITY READMISSION, FACILITATE RECOVERY, AND DEFER NURSING HOME PLACEMENT, 92 UNDUPLICATED HOMEBOUND SENIORS RECEIVED EMERGENCY MEALS ON WHEELS, 1,167 MEALS SERVED.


CASE MANAGEMENT:CASE MANAGERS COORDINATE WITH SENIORS (AND THEIR CAREGIVERS) TO IDENTIFY AND ARRANGE FOR THE SERVICES NEEDED TO SUPPORT THE INDIVIDUAL IN THEIR HOME. IN HOME SUPPORT INCLUDES, COMPANIONSHIP, HOMEMAKING, PERSONAL CARE, MEALS AND RESPITE. 131 OLDER ADULTS RECEIVED IN-HOME SERVICES; 100% OF CASE MANAGED CLIENTS WERE DEFERRED FROM FACILITY PLACEMENT.90 HOUSEHOLDS WERE ASSISTED THROUGH THE EMERGENCY HOME ENERGY ASSISTANCE FOR THE ELDERLY PROGRAM.INFORMATION AND REFERRAL FOR PROGRAMS AND SERVICES ARE AVAILABLE TO SENIORS. APPROXIMATELY 2,324 INDIVIDUALS RECEIVED ASSISTANCE THROUGH INFORMATION AND REFERRAL.


DIRECT ELDERLY CARE ADULT DAY CARE - PROVIDES COORDINATED HEALTH, SOCIAL, AND THERAPEUTIC ACTIVITIES IN A SUPERVISED GROUP SETTING FOR ADULTS WITH FUNCTIONAL AND/OR COGNITIVE IMPAIRMENTS. 96 UNDUPLICATED CLIENTS SERVED; 81% DEFERRED FROM NURSING HOME PLACEMENT.
