TO PROVIDE PRIMARY MEDICAL CARE TO THE CITIZENS OF FLORENCE. TARGET POPULATION IS THE MEDICALLY UNDERSERVED IN FLORENCE, CLARENDON, AND WILLIAMSBURG. HEALTH CARE IS PROVIDED TO THE UNINSURED AS WELL AS THOSE WITH GOVERNMENT AND PRIVATE INSURANCE. PROVIDED MEDICAL CARE SERVICES TO 54,116 PATIENTS RESULTING IN 277,370 PATIENT VISITS. SERVICE DELIVERY PLAN INCLUDES PRIMARY HEALTH CARE, DIAGNOSTIC LABORATORY AND RADIOLOGY, PREVENTATIVE HEALTH, URGENT CARE, PHARMACEUTICALS, MEDICAL REFERRALS, MEDICAL CASE MANAGEMENT, COUNSELING, TRANSPORTATION, OUTREACH, HEALTH EDUCATION, DENTAL CARE, SUBSTANCE ABUSE COUNSELING, NUTRITION COUNSELING AND MENTAL HEALTH TREATMENT.

PROVIDED MEDICAL, SUPPORTIVE AND EDUCATIONAL SERVICES TO PEOPLE LIVING WITH HIV/AIDS IN ORANGEBURG, CALHOUN, BAMBERG, AND AIKEN COUNTIES. 801 PATIENTS RECEIVED COMPREHENSIVE SPECIALTY CARE. PROGRAM SERVICES INCLUDE CASE MANAGEMENT, TRANSPORTATION, FOOD ASSISTANCE, BEHAVIORAL HEALTH COUNSELING, SUBSTANCE ABUSE COUNSELING AND DRUG ASSISTANCE PROGRAMS.
