STUDENTS WITH HEALTH INSURANCE OR MEDICAID COVERAGE WILL BE ASKED TO PROVIDE INFORMATION FOR BILLING SERVICES. STUDENTS WITHOUT HEALTHCARE COVERAGE WILL BE BILLED ON A SLIDING SCALE. NO STUDENT THAT HAS A SIGNED CONSENT FORM WILL BE TURNED AWAY FOR FAILING TO PAY OR LACK OF INSURANCE. THE CENTERS ARE OPEN MONDAY THROUGH FRIDAY DURING SCHOOL HOURS.
