HEALTH CARE FOR ALL IS THE HEART OF THE OAKLAND-BASED GRASSROOTS ORGANIZATION STREET LEVEL HEALTH PROJECT STREET LEVEL. INCORPORATED IN 2001, STREET LEVEL IS A CRITICAL ENTRY POINT INTO THE HEALTHCARE AND SOCIAL SERVICE SYSTEM FOR THOSE MOST OFTEN OVERLOOKED.

THROUGH A WHOLE PERSON CARE MODEL, THE HEALTH ACCESS PROGRAM SUPPORTS CLIENTS IN REDUCING THEIR INCIDENCE OF PREVENTABLE MALNUTRITION AND CHRONIC ILLNESS BY PROVIDING FREE HEALTH SCREENINGS AND NUTRITION CONSULTATIONS VIA PHONE. OUR HOTLINE IS OPEN 5 DAYS A WEEK TO CONNECT PATIENTS TO HEALTH-INSURANCE ENROLLMENT, PRIMARY CARE, HOUSING, AND OTHER HEALTH RELATED RESOURCES. IN 2021, WE PROVIDED REMOTE MEDICAL SERVICES TO 500 UNDUPLICATED PATIENTS DURING 523 HEALTH SCREENINGS. WE ALSO PROVIDED 932 MENTAL HEALTH CONSULTATIONS TO 215 PATIENTS AND 242 NUTRITION CONSULTATIONS TO 217 PATIENTS. THE TOP MOST COMMON CONDITIONS FOR 2021 WERE DIABETES TYPE II, DEPRESSION, ANXIETY, HYPERTENSION, AND CHEST PAIN. WE CONTINUE TO BE AN ENTRY POINT INTO THE HEALTHCARE SYSTEM FOR THOSE THAT MAY NOT OTHERWISE ACCESS THESE SERVICES.
THE WELLNESS AND PREVENTION PROGRAM PROVIDES HEALTH NAVIGATION AND REFERRAL SERVICES TO UNINSURED AND UNDERSERVED IMMIGRANTS IN THE BAY AREA. IT WORKS IN COORDINATION WITH OUR MUTUAL AID PROGRAM, WHICH PROVIDES WEEKLY DISTRIBUTION OF FRESH FRUITS AND VEGETABLES TO SINGLE ADULTS AND FAMILIES IN THE SURROUNDING NEIGHBORHOOD, AS WELL AS HOT AND NUTRITIOUS PREPARED MEALS OFFERED ALONGSIDE OUR PRODUCE. IN 2021, STREET LEVEL HEALTH PROJECT DISTRIBUTED 5669 FRESH PRODUCE BAGS, BENEFITING 11442 ADULTS AND 6428 CHILDREN. THESE FOOD DISTRIBUTIONS INCLUDED CONVERSATIONS WITH THE COMMUNITY ABOUT HEALTHY EATING, FOOD NUTRITION VALUE, AND EXPOSING CLIENTS TO NEW TYPES OF PRODUCE. THE WELLNESS AND PREVENTION PROGRAM ALSO PROVIDED 866 REFERRALS TO OTHER COMMUNITY BASED ORGANIZATIONS AND AGENCIES FOR NEEDS RELATING TO IMMIGRATION, HOUSING, EMPLOYMENT, AND FAMILY LAW, ENROLLMENT IN HEALTH INSURANCE PROGRAMS AND OTHER GOVERNMENT BENEFITS, AFFORDABLE HOUSING AND SHELTER SPACE, CONSULATE SERVICES, TAX PREPARATION, AND ADULT EDUCATION.
THE IMMIGRANT RIGHTS AND EMPOWERMENT PROGRAM OFFERS A SPACE FOR DAY LABORERS AND LOW WAGE WORKERS TO BUILD COMMUNITY, ORGANIZE, AND ADVOCATE FOR ISSUES IMPACTING THEIR COMMUNITY. THE OAKLAND WORKERS' COLLECTIVE HAS COMPONENTS: VOCATIONAL, TRAINING, LEADERSHIP DEVELOPMENT, AND EMPLOYMENT LINKAGE. IN 2021 WE PROVIDED 441 PLACEMENTS TO WORKERS PARTICIPATING IN THE PROJECT, MOREOVER, WE DEVELOPED RELATIONSHIPS WITH 131 NON-DUPLICATED EMPLOYERS, SOME OF WHOM RETAIN OWC WORKERS POST INITIAL PLACEMENT. THROUGH OUR STREET OUTREACH TO DAY LABORERS AT THE 6 DIFFERENT SITES WE PROVIDED OUTREACH AND EDUCATION AND MIN-WORKSHOPS ON TOPICS SUCH AS WORKPLACE HEALTH AND SAFETY, EMPLOYMENT RIGHTS, HEALTH AND WELLNESS TRAINING, KNOW YOUR RIGHTS, PROFESSIONALISM AT THE WORKPLACE AND COVID EDUCATION AND PREVENTION INCLUDING PROVIDING PPE. WE HAVE CONTINUED OUR COLLABORATION WITH CENTRO LEGAL DE LA RAZA AND CAUSA JUSTA: JUST CAUSE TO PROVIDE A WRAPAROUND SERVICE MODEL THAT RESPONDS TO THE BASIC NEEDS OF LOW WAGE WORKERS IN ALAMEDA COUNTY. WE HAVE ALSO CONTINUED COLLABORATION TO SUPPORT COMMUNITY EDUCATION AROUND THE ISSUE OF MINIMUM WAGE WITH SEVEN AGENCIES IN OAKLAND THROUGH THE FAIR LABOR OAKLAND COALITION. LASTLY, WE CONTINUED WITH OUR COLLABORATION WITH THE REGIONAL COALITION OF DAY LABORER ORGANIZATIONS IN NORTHERN CALIFORNIA AND WITH OUR PARTICIPATION WITH THE NATIONAL DAY LABORER ORGANIZING NETWORK.
