WCMSCHF'S FLAGSHIP PROGRAM, PROJECT ACCESS OF WAKE COUNTY (PA), HAS BEEN PROVIDING ACCESS TO DONATED SPECIALTY AND DIAGNOSTIC CARE FOR ELIGIBLE UNINSURED RESIDENTS OF WAKE AND FRANKLIN COUNTIES WITH HOUSEHOLD INCOMES BELOW 200% OF THE FEDERAL POVERTY LEVEL FOR 21 YEARS. PA ALSO NAVIGATES UNINSURED PATIENTS WITHOUT A PRIMARY CARE HOME TO AFFORDABLE OR FREE CARE AT ONE OF 12 PARTNERING SAFETY-NET CLINICS. PA STAFF ALSO ENGAGE WITH PROGRAM ENROLLEES TO ASSURE THAT PATIENTS COMPLETE CONNECTIONS WHILE HELPING BUILD AWARENESS AND ABILITY TO SELF-MANAGE THE HEALTHCARE LANDSCAPE.

PROJECT ACCESS OF WAKE & FRANKLIN COUNTIES (PA) - THE PA PROGRAM IS A COLLABORATIVE ALLOWING PARTNER ORGANIZATIONS TO PROVIDE MEDICAL CARE IN A COORDINATED FASHION FOR THE LOW-INCOME COMMUNITY WHILE FACILITATING ONGOING COMMUNICATION, COORDINATION OF SERVICES, ASSESSMENT OF COMMUNITY HEALTH NEEDS, IDENTIFICATION OF PRIORITIES, AND INITIATION OF WORKING PARTNERSHIPS AMONG PROVIDERS. THE PA PROGRAM IS A PRIVATE, NON-PROFIT PROGRAM THAT CONNECTS ELIGIBLE UNINSURED CLIENTS TO HIGH QUALITY MEDICAL SERVICES DONATED BY PHYSICIANS.
