TO SUPPORT THE LONG-TERM STRATEGIC INITIATIVES OF HOSPICE OF THE CHESAPEAKE INC WHICH INCLUDE 1)UNREIMBURSED CARE- COMMITTED TO PROVIDING ALL PATIENTS AND THEIR FAMILIES THE BEST POSSIBLE CARE REGARDLESS OF THEIR INABILITY TO PAY. 2)TRANSFORMING CARE-OFFER PATIENTS THE OPPORTUNITY TO TAKE ADVANTAGE OF A FULL RANGE OF SERVICES IN THE MANAGEMENT OF THEIR ILLNESS BY SUPPORTING THEM SOONER IN THEIR DIAGNOSIS AND PROVIDING GREATER ACCESSIBILITY TO NEW THERAPIES AS WELL AS THE OPPORTUNITY TO CONTINUE TRADITIONAL INTERVENTIONS WHILE STILL RECEIVING OUR COMPASSIONATE CARE. 3)THE LIFE CENTER-SPECIALIZING IN GRIEF, LOSS AND TRANSITION IN OUR COMMUNITY FOR INDIVIDUALS AND FAMILIES LIVING WITH AND AFFECTED BY ADVANCED ILLNESS, ACCIDENT, SUICIDE OR SUDDEN ILLNESS. 4)ADVANCEMENT/EHANCEMENT OF HOSPICE TEAM-WE ARE COMMITTED TO ENSURING THAT OUR TEAM HAS THE SPECIALIZED SKILLS AND KNOWLEDGE NEEDED TO PROVIDE THE MOST APPROPRIATE AND RELEVANT CARE POSSIBLE.
