OUR MISSION IS TO KEEP SENIORS SAFE, HEALTHY, AND IN THEIR HOMES AS LONG AS POSSIBLE. THROUGH UTILIZATION OF OUR TRANSPORTATION, IN-HOME CARE, AND LIFE ENRICHMENT SERVICES, WE FOCUS ON CARING FOR MIND, BODY, AND SPIRIT.

LIFE ENRICHMENT: PROVIDES A VARIETY OF SERVICES. SENIOR CASE MANAGEMENT PROVIDE FREE SENIOR-SPECIFIC EDUCATIONAL, SOCIALIZATION, AND EXERCISE ACTIVITIES. STATE HEALTH INSURANCE COUNSELORS ARE ON-SITE TO ASSIST WITH MEDICARE INSURANCE ENROLLMENT AND QUESTIONS, AS WELL AS PROCESS MEDICAL SAVINGS PLAN APPLICATIONS FOR PREMIUM SUPPLEMENT ASSISTANCE. MONEY MANAGEMENT STAFF ASSIST WITH REP PAYEE NEEDS. IN ADDITION, TAX PREPARATION SERVICES ARE PROVIDED. IN FISCAL 2020-21, 300 TAX RETURNS WERE COMPLETED, AND 116 MEDICAL INSURANCE CONSULTATIONS OCCURED.
TRANSPORTATION: PROVIDES TRANSPORTATION SERVICES TO OLDER ADULTS FOR MEDICAL, PHARMACY, NUTRITIONAL, BANKING, AND OTHER PERSONAL BUSINESS NEEDS. MODIFIED TRANSPORTATION SERVICES DURING COVID TO ALLOW FOR GROCERY DELIVERIES AND PARTNERSHIP WITH NORTHERN INDIANA FOOD BANK TO DELIVER FOOD TO HOMEBOUND ADULTS. WE PROVIDED 11,781 TRIPS FOR SENIORS IN FISCAL 2020-21.
