TO ENSURE THE BEST POSSIBLE HEALTH OUTCOMES, PATIENTS ARE STRONGLY ENCOURAGED TO UNDERGO ANNUAL HEALTHCARE CHECKUPS. HERE, AT SAN FERNANDO COMMUNITY HEALTH CENTER, OUR DOCTORS PROVIDE PRIMARY MEDICAL CARE TO PATIENTS OF ALL AGES WHO LIVE IN THE SAN FERNANDO VALLEY, LOS ANGELES COUNTY AREA. DURING THIS FISCAL YEAR, SFCHC PROVIDED 25,422 PRIMARY CARE MEDICAL VISITS TO MEMBERS OF THE COMMUNITY. MAINTAINING OPTIMAL ORAL HEALTH IS AN IMPORTANT PART OF OUR DAILY LIVES. AT SFCHC, OUR DOCTORS OFFER COMPREHENSIVE DENTAL SERVICES TO ENSURE THAT PATIENTS IN AND AROUND SAN FERNANDO VALLEY, CALIFORNIA AND THE SURROUNDING AREAS HAVE LASTING HEALTHY TEETH AND GUMS. DURING THIS FISCAL YEAR, SFCHC PROVIDED 6,478 DENTAL VISITS TO THE COMMUNITY. SAN FERNANDO COMMUNITY HEALTH CENTER PROVIDES INTEGRATED BEHAVIORAL HEALTH SERVICES, SUPPORTING A 'WHOLE PERSON' MODEL OF CARE TO ADDRESS THE MIND/BODY CONNECTION. OUR MENTAL HEALTH PROVIDERS OFFER INDIVIDUAL AND GROUP CARE TO ADOLESCENT AND ADULT PATIENTS THROUGHOUT THE SAN FERNANDO VALLEY, LOS ANGELES AREA. DURING THIS FISCAL YEAR, SFCHC PROVIDED 4,065 MENTAL HEALTH VISITS TO MEMBERS OF THE COMMUNITY. SFCHC OFFERS NUMEROUS HEALTH AND WELLNESS EDUCATION RESOURCES, INCLUDING ON-SITE AND VIRTUAL COOKING CLASSES, SEXUAL HEALTH INFORMATION, NUTRITION CONSULTATION, AND REMOTE PATIENT MONITORING FOR CHRONIC HEALTH CONDITIONS. PATIENTS THROUGHOUT THE SAN FERNANDO VALLEY, CALIFORNIA CAN FIND INDIVIDUALIZED TREATMENT THAT SUPPORTS AND MAINTAINS HEALTHY LIFESTYLES. DURING THIS FISCAL YEAR, SFCHC PROVIDED 2,228 NUTRITION, HEALTH EDUCATION, AND DIABETES TEACHING KITCHEN CLASSES TO 51 PARTICIPANTS. AND SFCHC PROVIDED 1 COMMUNITY EVENT REACHING 11 PARTICIPANTS DUE TO THE PANDEMIC CANCELING PUBLIC EVENTS IN MARCH 2020.
