MI CHOICE WAIVER IS A COMMUNITY BASED PROGRAM FOR ELIGIBLE INDIVIDUALS AGE 18 YEARS OR OLDER WHO MEET FINANCIAL AND MEDICAL REQUIREMENTS. BASED ON CLINICAL ASSESSMENT, PARTICIPANTS MAY RECEIVE PERSONAL CARE, RESPITE CARE, HOMEMAKING, PRIVATE DUTY NURSING, CHORE SERVICES, HOME DELIVERED MEALS, PERSONAL EMERGENCY RESPONSE SYSTEM, AND MANY OTHER SERVICES TO SUPPORT INDEPENDENT LIVING. THE APPROXIMATE NUMBER OF CLIENTS SERVED DURING THE YEAR WAS 535.
THE INFORMATION AND REFERRAL CALL CENTER ASSESSES CALLER'S NEEDS, REFERS CALLERS TO AVAILABLE AND APPROPRIATE COMMUNITY RESOURCES, AND PROVIDES PROGRAM ELIGIBILITY AND INTAKE INFORMATION.
