WE PROVIDE PERSONAL CARE SERVICES, WHICH INCLUDES ASSISTANCE WITH BATHING, GROOMING, DRESSING, PERSONAL HYGIENCE, MEDICATION REMINDERS, AND OTHER ACTIVITIES OF DAILY LIVING. THESE SERVICES ARE PROVIDED TO PEOPLE WHO ARE AT RISK OF HOSPITALIZATION OR INSTITUTIONALIZATION, SUCH AS THE ELEDERLY, CHRONICALLY ILL AND DISABLED. THE ORGANIZATION PARTICIPATES IN MEDICAID HCPS PROGRAMS TO PROVIDE COMPREHENSIVE PERSONAL CARE SERVICES FOR THE FRAIL ELDERLY, THE PHYSICALLY DISABLED, DEVELOPMENTALLY DISABLED, AND THOSE WITH TRAUMATIC BRAIN INJURY.
