ALSO KNOWN AS REMOTE PATIENT MONITORING, PERSONAL HEALTH AND MEDICAL DATA IS COLLECTED FROM THE PATIENT WHILE AT HOME AND TRANSMITTED TO THE HOME CARE AGENCY VIA A WEB PORTAL. THIS TYPE OF SERVICE ALLOWS A PROVIDER TO CONTINUE TO TRACK HEALTHCARE DATA FOR A PATIENT WHEN NOT IN THE HOME. THIS EFFECTIVE PATIENT MONITORING CAN HELP TO MAINTAIN THE PATIENT'S HEALTH IN THEIR HOME AND COMMUNITY, WITHOUT HAVING TO PHYSICALLY GO TO A PHYSICIAN OR EMERGENCY ROOM, BY REMINDING HE/SHE TO TAKE THEIR MEDICATIONS, MONITORING WEIGHT GAIN OR LOSS AND PROACTIVELY SENDING A NURSE TO THE HOME BEFORE A MEDICAL EMERGENCY ARISES.
