STATE FUNDED HOME CARE - PROVIDES COMMUNITY BASED SERVICES TO THE ELDERLY, INCLUDING CASE MANAGEMENT
ADULT FAMILY CARE AND PERSONAL CARE ATTENDANTS - PROVIDES FAMILY LIVING AS AN ALTERNATIVE TO INSTITUTIONAL CARE. THE PROGRAM MATCHES PARTICIPANTS WITH CAREGIVERS WHO PROVIDE 24-HOUR SUPPORT IN THEIR HOMES OR ALLOWS CONSUMERS TO SELECT, TRAIN AND EMPLOY THEIR OWN CAREGIVERS WITH SUPPORT FROM SOCIAL WORKERS AND NURSING STAFF.
SENIOR CARE OPTIONS - COMBINES HEALTH SERVICE FEE WITH SOCIAL SUPPORT SERVICES BY COORDINATING CARE AND SPECIALIZED GERIATRIC SUPPORT SERVICES, ALONG WITH RESPITE CARE FOR FAMILIES AND CAREGIVERS.
