THE HOME CARE PROGRAM PROVIDED CASE MANAGEMENT SERVICES FOR 2,703 OLDER ADULTS AND THE COORDINATION OF IN-HOME SERVICES TO 2,393 OLDER ADULTS IN FISCAL YEAR 2022.
