VFI COMMUNITY SERVICES PROVIDED PERSONAL ASSISTANT SERVICES (PAS) HOME CARE SERVICES UNDER THE CONSUMER EMPLOYER DELGATED MODEL EMPOWERING PROGRAM RECIPIENTS TO ASSIST IN THE HIRING AND MANAGING OF THEIR DIRECT CARE GIVERS. SUPPORT INCLUDED PROVIDING BILLING, PAYROLL, AND PAYROLL TAX SERVICES OF OVER 700 PARTICIPANTS. VFI COMMUNIITY SERVICES CONDUCTED PERSONAL CARE TRAINING AND CERTIFIED ALL PAS WORKERS FOR PERFORMANCE OF WORK AS PERSONAL CARE ATTENDANTS USING THE PRINCIPLES OF CONSUMER CONTROL AND INDEPENDENT LIVING.
VFI COMMUNITY SERVICES PROVIDED A NURSING HOME TRANSITION PROGRAM ESTABLISHED TO IDENTIFY AND TRANSITION INDIVIDUALS FROM NURSING HOMES TO RESIDENCES OF THEIR OWN. SERVICES INCLUDED IDENTIFICATION OF ELIGIBLE NURSING HOME RESIDENTS, COMMUNITY PLANNING WITH FAMILY AND SUPPORT NETWORKS, IDENTIFICATION AND PREPARATION OF HOUSING RESOURCES, THE PROVISION OF TANGIBLE ASSISTANCE SERVICES TO ACCOMPLISH THE RELOCATION TO COMMUNITY, LINKAGE TO COMMUNITY SUPPORT PROGRAMS, NON-MEDICAL TRANSPORTATION SERVICES, AND FOLLOW UP. PROGRAM RESULTED IN OVER 75 INDIVDUALS MOVING BACK INTO THE LOCAL COMMUNITY.
