UPSTATE FAMILY HEALTH CENTER, INC. (UFHC) IS A NONPROFIT ORGANIZATION PROVIDING HEALTH SERVICES AT TWO MAIN FACILITIES IN UTICA AND ROME, NY, FIVE SCHOOLBASED HEALTH CENTERS, AND VIA TELEMEDICINE. SERVICES ARE OFFERED BY 24 PROVIDERS, AND INCLUDE PRIMARY CARE, MENTAL HEALTH, PODIATRY, OCCUPATIONAL THERAPY, CHIROPRACTIC, SUBSTANCE ABUSE COUNSELING, OCCUPATIONAL THERAPY, TREATMENT FOR HEPETITISC, AND PARAMEDICINE. UFHC ALSO ADDRESSES THE SOCIAL DETERMINANTS OF HEALTH BY PROVIDING HOT MEALS, FOOD, TRANSPORTATION, INTERPRETIVE SERVICES, CLOTHING, SUPPLIES FOR INFANT CARE, SCHOOL BOOK BAGS, AND CRITICAL PHARMACEUTICALS.
