TO PROVIDE HEALTH CARE SERVICES BY OPERATING DIAGNOSTIC AND TREATMENT FACILITIES PURSUANT TO ARTICLE 28 OF THE PUBLIC HEALTH LAW OF NEW YORK STATE. THE CENTER OPERATES THREE CLINICS IN DUNKIRK, NEW YORK AND ONE CLINIC IN JAMESTOWN, NEW YORK. THE CENTER PROVIDES THESE AMBULATORY HEALTH CARE, WELLNESS, AND RELATED SERVICES TO ANYONE, REGARDLESS OF ABILITY TO PAY, RESIDING IN UNDER-SERVED AND OTHER DESIGNATED COMMUNITIES IN THE CHAUTAUQUA AND SURROUNDING REGION.

IN 2021, THE CHAUTAUQUA CENTER SERVED 12,203 INDIVIDUALS IN THE CHAUTAUQUA COUNTY REGION AREA. THE CENTER IMPROVED COMMUNITY HEALTH BY PROVIDING ACCESS TO QUALITY CARE AT AN AFFORDABLE PRICE, THROUGH THE PROMOTION OF HEALTHY LIFESTYLES AND HEALTH EDUCATION IN ORDER TO ACHIEVE HEALTH EQUITY. SCHEDULED OUTREACH IN THE COMMUNITY PROVIDES INDIVIDUALS AWARENESS OF THE SERVICES AVAILABLE AND SUPPORT WITH OBTAINING INSURANCE COVERAGE OF SLIDING FEE SCALE ARRANGEMENTS. THE CENTER HONORS RELATIONSHIPS WITH THOSE WE SERVE, THE STAFF, PROVIDERS, BOARD MEMBERS, VOLUNTEERS, OUR MANY COMMUNITY PARTNERS AND FRIENDS AS THEY ARE THE VEHICLE BY WHICH THE MISSION IS MAINTAINED. DURING THE YEAR, THE CENTER SERVICED OVER 53,000 VISITS.
