CARE PARTNERS OF COOK COUNTY PROVIDES CORRDINATION, END OF LIFE CARE, CAREGIVER SUPPORT, RESOURCE REFERRAL AS WELL AS VOLUNTEER ASSISTANCE WITH COMPANIONSHIP, RIDES, DELIVERIES AND CHORES TO OLDER ADULTS IN COOK COUNTY TO HELP THEM LIVE WELL AT HOME. IN 2022 CARE PARTNERS ASSISTED 93 CLIENTS WITH 1343 SERVICE UNITS OF SUPPORT SUCH AS COACHING CALLS, RIDES, VISITS, DELIVERIES OR HOURS OF CHORE HELP. CARE PARTNERS IS PRIMARILY VOLUNTEER BASED WITH OVER 50 ACTIVE, TRAINED VOLUNTEERS. CARE PARTNERS WORKS CLOSELY WITH OTHER AGING SERVICE PROVIDERS IN THE COUNTY TO MAXIMIZE SERVICES AND CONTINUITY OF CARE.
