PROVIDE HOSPICE, PALLIATIVE, AND BEREAVEMENT SERVICES

Describe the Organization's Program Activity:

HOSPICE CARE - THIS PROGRAM SUPPORTS THOSE WHO MAY BE IN THEIR LAST MONTHS OF LIFE AND PROVIDES PHYSICAL, EMOTIONAL, AND SPIRITUAL SUPPORT TO THE INDIVIDUAL AND FAMILY. CARE IS PROVIDED AT A HOSPICE RESIDENCE ON UNITY'S CAMPUS OR WHEREVER THE INDIVIDUAL LIVES, WHETHER A PRIVATE HOME, NURSING HOME, ASSISTED LIVING FACILITY, HOSPITAL, OR UNITY'S JACK AND ENGRID MENG RESIDENCE. UNITY PROVIDES CARE TO ALL PATIENTS, REGARDLESS OF ABILITY TO PAY.


CARE COMPANIONS, LLC - UNITY RECOGNIZES THAT MOST PEOPLE WISH TO LIVE INDEPENDENTLY IN THEIR HOME AND OFFERS A WIDE RANGE OF PERSONALIZED, NON-MEDICAL SERVICES TO MEET THEIR EVER-CHANGING NEEDS. EXAMPLES OF SERVICES PROVIDED ARE: PERSONAL ASSISTANCE, COMPANIONSHIP, AND HOUSEKEEPING TASKS.


GRIEF SERVICES - THIS PROGRAM ENSURES THAT, FOLLOWING THE DEATH OF A FRIEND OR LOVED ONE, NO ONE HAS TO WALK ALONE. UNITY PROVIDES FOLLOW-UP CARE--MAILINGS, PHONE CALLS, PERSONAL VISITS, AND GROUP SUPPORT--TO MEET THE INDIVIDUAL NEEDS OF EACH PERSON. A MINIMUM THIRTEEN MONTHS OF CARE ALLOWS INDIVIDUALS TO PASS THROUGH ALL THE "FIRSTS" WHILE RECEIVING THE REASSURANCE THAT, ALTHOUGH DIFFICULT, GRIEF IS A NATURAL PROCESS THAT IS DIFFERENT FOR EVERY PERSON. UNITY'S BEREAVEMENT CARE IS OFFERED TO ALL RESIDENTS OF NORTHEAST WISCONSIN, REGARDLESS OF PRIOR AFFILIATION TO UNITY.


SUPPORTIVE CARE MANAGEMENT, LLC - UNITY NOTICED A GAP IN CARE FOR INDIVIDUALS WHO WERE FACING A SERIOUS ILLNESS AND DID NOT QUALIFY FOR HOSPICE. MANY OF THESE INDIVIDUALS MAY HAVE BEEN RECEIVING CURATIVE TREATMENTS OR MAY HAVE HAD A LIMITING BUT INDETERMINATE PROGNOSIS. WHILE THESE INDIVIDUALS FACED THE BURDEN OF BALANCING MULTIPLE APPOINTMENTS, TREATMENTS, MEDICATIONS, DECISIONS, AND OTHER HEALTH MATTERS, THEY DID NOT HAVE ONE CONSISTENT PLAN OF CARE MANAGED BY ANY ONE PROVIDER. THIS IS WHEN UNITY ESTABLISHED ITS PALLIATIVE CARE PROGRAM TO ADDRESS NOT ONLY THE PHYSICAL SYMPTOMS OF A SERIOUS ILLNESS OR CONDITION BUT THE EMOTIONAL, SOCIAL, AND SPIRITUAL CONCERNS AS WELL.
