IN 2021, THROUGH CCN'S COLLABORATIVE EFFORTS WITH OUR NETWORK OF OVER 100 PARTNERS, WE HAVE CONTINUED TO ENGAGE THE MEDICAID MEMBERS THROUGH DEVELOPMENT OF THE NEW SOCIAL IMPACT PILOT PROGRAM, LEVERAGING OUR POPULATION HEALTH AND CARE MANAGEMENT PLATFORMS TO CREATE NETWORKS OF CARE AROUND TARGETED POPULATIONS. METRIC INFORMATION PROVIDED BY THE NYS DEPARTMENT OF HEALTH HAS SHOWN THESE EFFORTS HAVE IMPROVED HEALTH OUTCOMES IN OUR REGION. ADDITIONALLY, IN 2021, CCN CONTINUED THE IMPLEMENTATION OF POPULATION HEALTH AND CARE MANAGEMENT SOFTWARE PLATFORMS WITH 12 PARTNERS ENROLLED IN THE CARE MANAGEMENT PLATFORM AND 68 PARTNERS ENROLLED IN THE POPULATION HEALTH PLATFORM.
