Describe the Organization's Program Activity:
NUTRITION: SERVES HOT MEALS TO PERSONS 60 AND OVER AT MORE THAN 19 CONGREGATE SITES IN SOUTHEAST SOUTH DAKOTA INCLUDING HOME DELIVERED MEALS TO HOME BOUND PEOPLE.
DAY BREAK: PROVIDES SUPERVISION AND PERTINENT DAY CARE TO PERSONS 55 AND OLDER WITH DEMENTIA RELATED ILLNESSES. PARTICIPANTS ALSO COULD BE AFFECTED WITH PHYSICAL OR EMOTIONAL CONCERNS THAT PUT THEM AT RISK OF LOSING THEIR INDEPENDENCE OR BEING AT RISK WHEN LEFT ALONE.
CENTER ACTIVITIES: PROGRAMS FOR RECREATION, EDUCATION, TRANSPORTATION, WORKERS ON WHEELS, AND BRIDGES EMPLOYMENT RESOURCES FOR SENIORS.
BERGELAND APARTMENTS IS A 45-UNIT APARTMENT COMPLEX LOCATED IN THE CITY OF SIOUX FALLS, SOUTH DAKOTA. THE APARTMENT COMPLEX PROVIDES HOUSING FOR ELDERLY, LOW-INCOME INDIVIDUALS. SHINE (SENIOR HEALTH INFORMATION AND INSURANCE EDUCATION): PROVIDES ADVOCACY, INFORMATION, EDUCATION, AND ASSISTANCE TO CONSUMERS ON MEDICARE. STAFF AND TRAINED VOLUNTEER COUNSELORS ARE AVAILABLE STATEWIDE TO ASSIST PEOPLE, FREE OF CHARGE, WITH PROBLEMS OR QUESTIONS REGARDING THEIR MEDICARE BENEFITS.
