THIS MISSION IS ACCOMPLISHED THROUGH FEDERAL AND STATE GRANTS AWARDED TO CONTRACTORS WHO PROVIDE SOCIAL SERVICES, CAREGIVER SUPPORT SERVICES, NUTRITION PROGRAMS AND CHRONIC DISEASE MANAGEMENT ACTIVITIES.
