TO HELP OLDER ADULTS AGE SUCCESSFULLY WHILE MAINTAINING THEIR DIGNITY AND INDEPENDENCE IN THE COMMUNITY THROUGH INFORMED CHOICES. THIS MISSION IS ACCOMPLISHED THROUGH FEDERAL AND STATE GRANTS AWARDED TO CONTRACTORS WHO PROVIDE SOCIAL SERVICES, CAREGIVER SUPPORT SERVICES, NUTRITION PROGRAMS AND CHRONIC DISEASE MANAGEMENT ACTIVITIES.

AWARDING AND MONITORING FEDERAL AND STATE GRANTS FOR SENIOR NUTRITION PROGRAMS, INCLUDING CONGREGATE AND HOME-DELIVERED; IN-PERSON ASSISTANCE TO SENIOR AND THEIR CAREGIVERS FOR SENIORS DESIRING TO RETURN HOME FROM AN INSTITUTIONAL-BASED SETTING; AND PROVIDING INFORMATION AND IN-DEPTH ASSISTANCE THROUGH THE 'SENIOR LINKAGE LINE' TO THE 11 COUNTY REGION.
