THE SUPPORT PROVIDED INCLUDED MEDICATION MONITORING, NURSING, TRANSPORTATION, LEISURE/RECREATION ACTIVITIES, GROUPS, REPRESENTATIVE PAYEE, JOB RELATED SERVICES, AND OTHER GENERAL DAILY LIVING SUPPORT SERVICES. PROVIDED HOUSING TO APPROXIMATELY 37 COMMUNITY CLIENTS DIAGNOSED WITH SERIOUS MENTAL ILLNESS OF WHICH 20 RECEIVED 24 HOUR SUPERVISION.
