HOUSING 1989 IS A NOT-FOR-PROFIT CORPORATION LOCATED IN QUINCY, IL. HOUSING 1989'S MISSION IS TO PROVIDE SAFE AND AFFORDABLE COMMUNITY-BASED HOUSING TO ADULTS WHO HAVE A SEVERE MENTAL ILLNESS OR A DEVELOPMENTAL DISABILITY. HOUSING 1989 OWNS A 15 BED GROUP HOME AND A 15 UNIT APARTMENT COMPLEX IN ADAMS COUNTY, ILLINOIS. THESE FACILITIES ARE HOUSING AND URBAN DEVELOPMENT (HUD) SPONSORED, MAKING IT POSSIBLE FOR RESIDENTS TO RESIDE IN THE HOMES ACCORDING TO THEIR INCOMES. TRANSITIONS OF WESTERN IL., A NOT-FOR-PROFIT PROVIDER OF MENTAL HEALTH, REHABILITATION, AND EDUCATIONAL SERVICES, PROVIDES STAFFING AND SERVICES IN THE GROUP HOME AND TO RESIDENTS OF THE APARTMENTS OWNED BY HOUSING 1989. DURING FY22, 38 DIFFERENT ADULTS WITH A SEVERE MENTAL ILLNESS OR A DEVELOPMENTAL DISABILITY RESIDED IN THE FACILITIES OWNED BY HOUSING 1989. THESE 38 RESIDENTS RECEIVED CARE AND SUPERVISION/SUPPORT FROM TRANSITIONS' STAFF. RESIDENTS LEARNED SKILLS OF INDEPENDENT LIVING - SKILLS SUCH AS GROOMING/HYGIENCE SKILLS, SOCIAL SKILLS, LAUNDRY SKILLS, AND RECREATIONAL/LEISURE SKILLS - THROUGH THE MILIEU IN THE GROUP HOME AND FROM SKILLS TRAINING PROVIDED BY STAFF. STAFF IN THE GROUP HOME PROVIDED MEALS AND ASSISTED RESIDENTS, AS NEEDED, WITH PERSONAL NEEDS, SUCH AS MEDICATION MANAGEMENT, TRANSPORTATION IN THE COMMUNITY, APPOINTMENTS WITH OTHER PROVIDERS (MEDICAL AND SOCIAL SERVICES), SELF-CARE. WITH AFFORDABLE HOUSING PROVIDED BY HOUSING 1989 AND THE SUPERVISION AND SUPPORTS PROVIDED BY TRANSITIONS, ADULTS WITH A SEVERE MENTAL ILLNESS OR A DEVELOPMENTAL DISABILITY LIVE SUCCESSFULLY IN THE COMMUNITY, CLOSE TO FAMILY AND FRIENDS.
