OREGON HEALTH PLAN PROGRAM: THROUGH OUR CONTRACTED RELATIONSHIPS WE PROVIDE ACCESS FOR LOW-INCOME YAMHILL COUNTY, OREGON HEALTH PLAN MEMBERS. OUR WORK INVOLVES COORDINATING QUALITY HEALTHCARE FOR MEMBERS AND CONTRACTING WITH PUBLIC AND PRIVATE PROVIDERS TO PROVIDE HEALTHCARE SERVICES. YAMHILL COMMUNITY CARE ORGANIZATION IS A COMMUNITY ORGANIZATION OPERATED FOR THE BENEFIT OF THE POOR AND VULNERABLE IN YAMHILL COUNTY, OREGON. AS A COMMUNITY ORGANIZATION, WE ARE EMPOWERED BY THE BOARD OF DIRECTORS AND STAKEHOLDER REPRESENTATIVES TO DIRECT REVENUE AND COMMUNITY INVESTMENTS INTO BOLSTERING THE HEALTHCARE DELIVERY SYSTEM AND MAKING THE NECESSARY CHANGES TO MEET BOTH THE LEGISLATION AND COMMUNITY NEED.
