SERVICES OFFERED INCLUDE TRANSPORTATION, INFORMATION AND REFERRAL, VOLUNTEER/EDUCATION, CASE MANAGEMENT, CAREGIVER SUPPORT, ADULT PROTECTIVE SERVICES, AND OTHER ACTIVITIES.

THE COMMUNITY CARE PROGRAM ("CCP") OFFERS A VARIETY OF SERVICES TO HELP OLDER ADULTS RETAIN INDEPENDENCE AND REMAIN AT HOME. CCP SERVES APPROXIMATELY 350 PEOPLE. THE PROGRAM HAS BEEN IN EXISTENCE FOR OVER 15 YEARS.
THE TRANSPORTATION PROGRAM IS SET UP TO PROVIDE BUS OR VAN TRANSPORATION SERVICES TO SENIOR CITIZENS.
TITTLE III-B OF OLDER AMERICANS ACT SERVES APPROXIMATELY 3,000 PEOPLE. THE PROGRAM HAS BEEN IN EXISTENCE FOR OVER 15 YEARS.
