HEALTHY START CARE COORDINATION - PROFESSIONAL CASE MANAGEMENT OF PREGNANT WOMEN AND INFANTS WHO ARE AT RISK FOR ADVERSE OUTCOMES (FETAL DEATH, PREMATURITY, LOW BIRTH WEIGHT, AND INFANT DEATH). INCLUDES HOME VISITING, EDUCATION, SUPPORT, AND REFERRALS TO ADDITIONAL SPECIALIZED SERVICES.
HEALTHY START WRAP AROUND SERVICES - SPECIALIZED SERVICES FOR PREGNANT WOMEN AND INFANTS IN HEALTHY START WHO NEED ADDITIONAL EDUCATION AND COUNSELING FOR RISKS THAT COULD LEAD TO POOR BIRTH OUTCOMES.
