CARE COORDINATION: PROFESSIONAL CASE MANAGEMENTOF PREGNANT WOMEN AND INFANTS WHO ARE AT RISK FOR ADVERSE OUTCOMES (FETAL DEATH, PREMATURITY, LOW BIRTH WEIGHT, AND INFANT DEATH). INCLUDES HOME VISITING, EDUCATION, SUPPORT, AND REFERRALS TO ADDITIONAL SPECIALIZED SERVICES.
OTHER: SAVE MY LIFE IS A FREE, CULTURALLY SENSITIVE EDUCATION AND SUPPORT PROGRAM FOR WOMEN OF COLOR IN SARASOTA. CLASSES ARE AVAILABLE IN-HOME, ON-SITE, AND IN VARIOUS LOCATIONS THROUGHOUT NORTH SARASOTA. MOM CARE HELPS MEDICAID ELIGIBLE PREGNANT WOMEN ACCESS EARLY PRENATAL CARE AND PROVIDE IMPORTANT INFORMATION AND RESOURCES TO HELP THEM HAVE HEALTHY PREGNANCIES.
FATHERHOOD: THE FATHERHOOD PROGRAM OFFERS EDUCATION, TRAINING, AND SUPPORT TO HELP MEN BECOME THE RESPONSIBLE ENGAGED AND EMPOWERED FATHERS THEIR CHILDREN NEED THEM TO BE.
FIMR - THE FIMR CASE REVIEW TEAM (CRT) EXAMINES FETAL AND INFANT DEATHS IN ORDER TO ACHIEVE A BETTER FUTURE FOR ALL CHILDREN AND FAMILIES.
