PLANS, COORDINATES AND PROVIDES COMMUNITY HEALTH SERVICES TARGETING INDIVIDUALS IN THE RIVER REGION THROUGH FOUR PRIMARY FOCUS AREAS: (1) THE BOARD MEETS ON A REGULAR BASIS AND MEMBERS WORK TOGETHER TO PLAN AND COORDINATE SERVICES TO THE TARGET POPULATION PROVIDED THROUGH THE "SAFETY NET" PROVIDERS AND TO HELP REDUCE THE COST OF HEALTH CARE TO THE COMMUNITY; (2) AGENCY PROVIDES WELLNESS CASE MANAGEMENT SERVICES TO THE TARGET POPULATION, INCLUDING LINKAGE TO A MEDICAL HOME FOR AFFORDABLE PRIMARY CARE SERVICES, ACCESS TO FREE MEDICATIONS AND OTHER COMMUNITY RESOURCES, AND CHRONIC DISEASE SELF-MANAGEMENT EDUCATION TO ASSIST MEMBERS OF THE TARGET POPULATION TO MANAGE CHRONIC DISEASES SUCH AS DIABETES, HEART DISEASE, ASTHMA, AND OBESITY; (3) PROVIDES COMMUNITY HEALTH INTERVENTIONS TO IMPROVE ACCESS TO HEALTHY FOODS IN CORNER STORES AND COMMUNITY GARDENS AND ACCESS TO CLINICAL AND COMMUNITY LINKAGES FOR LOW-INCOME MINORITY POPULATIONS IN THE COMMUNITY AND (4) PROVIDES CONTINUING EDUCATION OPPORTUNITIES FOR HEALTH CARE PROFESSIONALS.
