PRESBYTERIAN SENIORCARE AT HOME WAS LAUNCHED IN 2010 TO MEET THE NEEDS OF OLDER ADULTS TO RECEIVE CARE AND SERVICES IN THEIR HOME. IT SOLD ITS PRIVATE DUTY SERVICE BUSINESS IN 2017, AND NOW PROVIDES MEDICAL HOME HEALTH SERVICES IN THE HOME. PRESBYTERIAN SENIORCARE AT HOME RECEIVED MEDICARE CERTIFICATION IN SEPTEMBER, 2016 TO FURTHER ENHANCE CHOICES FOR CARE. AS AN AFFILIATE OF PRESBYTERIAN SENIORCARE, THE ADDITION OF THE HOME HEALTH AGENCY WAS AN IMPORTANT STEP IN ENHANCING THE CONTINUUM OF CARE PROVIDED THROUGH THE PRESBYTERIAN SENIORCARE REGIONAL NETWORK OF OFFERINGS IN ORDER TO MEET A COMPLETE RANGE OF NEEDS.THROUGH ITS MEDICARE CERTIFIED HOME HEALTH AGENCY, PRESBYTERIAN SENIORCARE AT HOME PROVIDES SKILLED NURSING, PHYSICAL AND OCCUPATIONAL SERVICES, SOCIAL WORK SERVICES AND HOME HEALTH AIDES. THESE SERVICES ARE COVERED BY MEDICARE FOR ELIGIBLE PATIENTS WHO ARE UNABLE TO LEAVE THEIR HOMES EXCEPT WITH GREAT HARDSHIP AND WHO NEED SKILLED NURSING CARE OR THERAPY TO TREAT EXISTING CHRONIC MEDICAL CONDITIONS. IN 2021, PRESBYTERIAN SENIORCARE AT HOME PROVIDED HOME HEALTH SERVICES FOR APPROXIMATELY 550 CLIENTS, WITH APPROXIMATELY 12,500 VISITS. PRESBYTERIAN SENIORCARE AT HOME CURRENTLY IS RECOGNIZED IN UPMC, HIGHMARK AND AETNA/COVENTRY MANAGED CARE NETWORKS.IN 2021, DEMENTIA 360, A NEW PROGRAM DEVELOPED BY PRESBYTERIAN SENIORCARE, WAS TRANSFERRED TO PRESBYTERIAN SENIORCARE AT HOME. DEMENTIA 360 PROVIDES CARE COORDINATION ASSISTANCE TO FAMILY MEMBERS AND CARETAKERS OF THOSE LIVING WITH DEMENTIA. PRESBYTERIAN SENIORCARE AT HOME WAS DEVELOPED AND CONTINUES TO BE OPERATED BY PRESBYTERIAN SENIORCARE.ESTABLISHED ON JANUARY 25, 1928 IN THE COMMONWEALTH OF PENNSYLVANIA, PRESBYTERIAN SENIORCARE IS A NOT-FOR-PROFIT, CHARITABLE ORGANIZATION WHICH PROVIDES FACILITIES, SERVICES AND PROGRAMS FOR ENHANCING THE QUALITY OF LIFE OF OLDER ADULTS. PRESBYTERIAN SENIORCARE GIVES BACK TO ITS LOCAL COMMUNITIES INCLUDING PROVIDING CHARITABLE CARE TO SENIORS IT SERVES THAT NO LONGER HAVE THE MEANS TO PAY, HAS APPROXIMATELY 2,000 TEAM MEMBERS WHO SERVE MORE THAN 6,500 OLDER ADULTS ANNUALLY THROUGH 53 COMMUNITIES/SERVICE LINES ACROSS 10 WESTERN PENNSYLVANIA COUNTIES, AS WELL THROUGH OUR AT HOME PROGRAMS OF ENVISAGE AND PRESBYTERIAN SENIORCARE HOME HEALTH. PRESBYTERIAN SENIORCARE GIVES BACK TO ITS LOCAL COMMUNITIES THROUGH PROMOTION OF HEALTH AND WELLNESS EFFORTS, EDUCATION AND TRAINING PROGRAMS, FINANCIAL AND IN-KIND DONATIONS, AND SPECIAL PROJECTS. GUIDED BY ITS FAITH-BASED NOT-FOR-PROFIT MISSION, PRESBYTERIAN SENIORCARE PROVIDED APPROXIMATELY $3 MILLION IN BENEVOLENT CARE ANNUALLY TO RESIDENTS IN OUR PERSONAL CARE/ASSISTED LIVING COMMUNITIES WHO NO LONGER HAVE THE MEANS TO PAY, AND $27 MILLION IN UNCOMPENSATED CARE TO RESIDENTS IN OUR NURSING FACILITIES WHO NO LONGER HAVE THE MEANS TO PAY.
