TO PROVIDE COMMUNITY BASED PRIMARY AND PREVENTATIVE MEDICAL AND DENTAL CARE SUBSIDIZED WITH GRANT MONEY BASED ON ABILITY TO PAY FOLLOWING THE FEDERAL GRANT'S GUIDELINES. THERE WERE A TOTAL OF 26,243 ENCOUNTERS FOR THE NUMBER OF PATIENTS SERVED DURING THE YEAR. UTILIZING AN INTEGRATED MODEL OF CARE BEHAVIORAL HEALTH CONSULTANTS CAN MEET WITH PATIENTS IN CRISIS FOR A BRIEF INTERVENTION AT THE TIME OF ANY APPOINTMENT AND MEET MENTAL HEALTH AS WELL AS THE PHYSICAL NEEDS OF THE PATIENT. SPRECTRA HEALTH'S INTEGRATED MODEL OF CARE WHICH ADDRESSES MORE THAN "JUST" BASIC HEALTHCARE NEEDS BUT ALSO ADDRESSES SOCIAL DRIVERS OF HEALTH WORKING HAND IN HAND WITH OUR OWN ON SITE SOCIAL SERVICE CASE WORKERS.
