TO PROVIDE COMPREHENSIVE PRIMARY AND PREVENTATIVE HEALTHCARE SERVICES THAT WILL IMPROVE THE HEALTH AND STATUS AND DECREASE HEALTH DISPARITIES OF THE MEDICALLY UNDERSERVED POPULATION IN RAVALLI COUNTY, MONTANA. SAPPHIRE COMMUNITY HEALTH SERVED APPROXIMATELY 2,100 INDIVIDUALS IN 2021 PROVIDING BASIC SERVICES INCLUDING GENERAL PRIMARY MEDICAL CARE, DOT AND SPORTS PHYSICALS, IMMUNIZATIONS AND WELL CHILD VISITS, VACCINATIONS, SCREENINGS FOR CANCER, ON-GOING CARE FOR CHRONIC ILLNESSES, COLORECTAL SCREENING, DERMATOLOGY PROCEDURES, AND PROVIDIDNG SOCIAL SERVICE INFORMATION AND REFERRALS.
