MERAKEY DELAWARE COUNTY OFFERS A BORAD CONTINUUM OF SERVICES FOR INDIVIDUALS WITH BEHAVIORAL HEALTH NEEDS ALONG WITH RESIDENTIAL SERVICES FOR INDIVIDUALS WITH INTELLECTUAL AND DEVELPMENTAL DISABILITIES. SEVERAL OF THE BEHAVIORAL HEALTH PROGRAMS PROVIDE INTEGRATED AND COORDINATED SERVICES AS A CERTIFIED COMMUNITY BEHAVIORAL HEALTH CENTER THROUGH A SAMHSA GRANT. OUTPATIENT SERVICES ARE AVAILABLE THROUGH OPEN ACCESS, PROVIDING 75-100 INDIVIDUALS PER MONTH WITH AN ASSESSMENT THAT IS THE GATEWAY TO MENTAL HEALTH TREATMENT. SUPPORT OF INDIVIDUALS THROUGH COMMUNITY-BASED PROGRAMS SUCH AS CASE MANAGEMENT AND PSYCHIATRIC REHABILITATION SERVICES. IN FY 2021, THESE PROGRAMS FOCUSED ON THE WELLNESS OF THE INDIVIDUALS SERVED BY PROVIDING REGULAR WELL-CHECKS, REMOTE GROUP INTERACTION, AND THE CONNECTION WITH COMMUNITY SERVICES THAT MEET IDENTIFIED NEEDS. THE TEAMS PROVIDED NURSE NAVIGATION SERVICES THAT BRIDGE THE GAP BETWEEN PHYSICAL HEALTH PROVIDERS, BEHAVIORAL HEALTH SUPPORTS, AND COMMUNITY RESOURCES. ADDITIONALLY, CERTIFIED PEER SPECIALISTS HELPED THE INDIVIDUALS DEVELOP SELF-ADVOCACY SKILLS, SELF-IMPROVEMENT STRATEGIES, AND THE DEVELOPMENT OF ROLES IN THE COMMUNITY.IN ADDITION TO THE COMMUNTIY-BASED SERVICES, MERAKEY DELAWARE COUNTY PROVIDES RESIDENTIAL SERVICES TO INDIVIDUALS WITH BEHAVIORAL HEALTH NEEDS THROUGH LONG TERM STRUCTURED RESIDENCES, COMMUNITY RESIDENTIAL REHABIILTATION, AND ASSISTED LIVING SITES. THESE RESIDENCES FOCUSED ON MAINTAINING THE SAFETY, WELLNESS, AND BEHAVIORAL HEALTH OF THE INDIVIDUALS SERVED THROUGH THE STRESSES OF THE PANDEMIC. THESE PROGRAMS WORKED TO SUPPORT INDEPENDENT, HEALTHY DECISIONMAKING IN ORDER TO MOVE THE INDIVIDUALS CLOSED TO LIVING INDEPENDENTLY WITH MINIMAL SUPPORTS.MERAKEY DELAWARE COUNTY'S RESIDENTIAL SERVICES FOR INDIVIDUALS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILTIES PROVIDED CONTINUING SUPPORT FOR RESIDENTS THROUGH THE STRESSES OF THE PANDEMIC.
